摘要
探讨血清C-反应蛋白(CRP)、白蛋白(Alb)、前白蛋白(PA)、肿瘤坏死因子-α(TNF-α)水平变化与抑郁症患者汉密尔顿抑郁量表(HAMD)评分的关联性。方法 选取(2017年12月~2019年11月)我院抑郁症患者91例为研究组,同期健康体检者42例为对照组,均测定血清CRP、Alb、PA、TNF-α水平,对比两组血清CRP、Alb、PA、TNF-α水平;并对抑郁症患者进行HAMD评分,给予盐酸文拉法辛缓释片治疗,比较不同HAMD评分患者、治疗3个月后有效与无效患者血清CRP、Alb、PA、TNF-α水平,分析血清CRP、Alb、PA、TNF-α水平与HAMD评分的关联性。结果 研究组血清CRP、TNF-α水平高于对照组,血清Alb、PA水平低于对照组(P<0.05);不同HAMD评分患者血清CRP、Alb、PA、TNF-α水平存在显著差异(P<0.05);经Pesrson分析,血清CRP(r=0.651)、TNF-α(r=0.477)水平与HAMD评分呈正相关,血清Alb(r=-0.782)、PA(r=-0.419)与HAMD评分呈负相关(P<0.05);治疗3个月后有效患者血清CRP、TNF-α水平低于无效患者,血清Alb、PA高于无效患者(P<0.05)。结论 血清CRP、TNF-α在抑郁症患者中呈高表达水平,与抑郁症患者HAMD评分呈正相关,血清Alb、PA在抑郁症患者中呈低表达水平,与抑郁症患者HAMD评分呈负相关,检测血清CRP、TNF-α、Alb、PA水平,可根据其水平变化评估抑郁症患者病情程度,为临床诊治提供有效参考。
Abstract
Objective To investigate the correlation between changes in serum C-reactive protein (CRP), albumin (Alb), and prealbumin (PA) levels and the Hamilton Depression Scale (HAMD) score in patients with depression. Methods From December 2017 to November 2019, 91 patients with depression in our hospital were selected as the study group, and 42 healthy patients who were examined during the same period were used as the control group. All serum CRP, Alb, PA, and tumor necrosis factor-α (TNF-α) levels were measured, and serum CRP, Alb, PA, and TNF-α levels were compared between the two groups; HAMD scores were given to patients with depression, and venlafaxine hydrochloride sustained-release tablets were given, the serum CRP, Alb, PA, and TNF-α levels of patients with different HAMD scores and those who were effective and ineffective after 3 months of treatment were compared, and the correlation between serum CRP, Alb, PA, and TNF-α levels and HAMD scores was analyzed. Results The levels of serum CRP and TNF-α in the study group were higher than those in the control group, and the levels of serum Alb and PA were lower than those in the control group (P<0.05); there were significant differences in serum CRP, Alb, PA and TNF-α levels in patients with different HAMD scores (P<0.05); according to Pesrson analysis, serum CRP (r=0.651) and TNF-α (r=0.477) levels were positively correlated with HAMD scores, while serum Alb (r=-0.782) and PA (r=-0.419) were negatively correlated with HAMD scores (P<0.05); after 3 months of treatment, the serum CRP and TNF-α levels of patients who were effective were lower than those of ineffective patients, and the serum Alb and PA were higher than those of ineffective patients (P<0.05). Conclusion Serum CRP and TNF-α are highly expressed in patients with depression, and are positively correlated with HAMD scores in patients with depression, serum Alb and PA are lowly expressed in patients with depression, and negatively correlated with HAMD scores in patients with depression. Detection of serum CRP, TNF-α, Alb, PA levels can be used to assess the severity of depression in patients with depression, providing an effective reference for clinical diagnosis and treatment.
