卒中后抑郁患者血清TNF-α水平的特点及其与认知功能之间的相关性

摘要
目的:探讨卒中后抑郁患者用药前TNF-α水平的特点及其与认知功能之间的相关性。方法:研究组:卒中后半年内抑郁发作的患者42例。对照组:健康成人40例。应用ELISA法分别对研究组和对照组进行TNF-α血清水平的检测;采用剑桥神经心理测试(CANTAB)中的工作记忆和策略运用(SWM),分别对研究组和对照组进行认知功能测试,内容包括组内搜索错误、组间搜索错误、策略运用和平均反应时间。结果:治疗前,卒中后抑郁患者血清TNF-α水平高于对照组,差异有统计学意义(P<0.05);认知测试(SWM)中,研究组的组内搜索错误、组间搜索错误研究组、策略运用研究组均高于对照组,差异有统计学意义(P<0.05);相关性分析发现,研究组血清TNF-α水平与SWM组内搜索错误、组间搜索错误、策略运用和平均反应时间均呈正相关(P<0.01)。结论:本研究发现,卒中后抑郁患者血清TNF-α水平升高、认知功能下降,提示TNF-α可能参与了卒中后抑郁的发生,相关性分析发现,TNF-α水平可能介导了认知功能下降。
Abstract
Objective:To explore the characteristics of TNF-α levels and their correlation with cognitive function in patients with post-stroke depression. Methods: Study group: 42 patients with depressive episodes within six months after stroke. Control group: 40 healthy adults. ELISA was used to detect serum TNF-α levels in the study group and the control group. Working memory and strategy application (SWM) in the Cambridge Neuropsychological Test (CANTAB) were used to test cognitive function in the study group and the control group, respectively. The content includes search errors within groups, search errors between groups, strategy application, and average response time. Results: Before treatment, the levels of serum TNF-α in patients with post-stroke depression were higher than those in the control group, and the difference was statistically significant (P <0.05). In the cognitive test (SWM), search errors within the group and search errors between groups The study group and strategy application study group were higher than the control group, and the difference was statistically significant (P <0.05). Correlation analysis found that serum TNF-α levels in the study group and search errors within the SWM group, search errors between groups, and strategy application There was a positive correlation with the average response time (P <0.01). Conclusion: This study found that serum TNF-α levels increased and cognitive function decreased in patients with post-stroke depression, suggesting that TNF-α may be involved in the occurrence of post-stroke depression. Correlation analysis found that TNF-α levels may mediate cognitive decline.
