摘要
【摘要】 探讨右美托咪定在抑郁症患者无抽搐电休克(modified electroconvulsive therapy,MECT)治疗中的应用。方法 前瞻性选择2021年8月~2022年8月我院收治的需接受MECT的患者106例以随机数字表法分观察与对照组,各53例。观察组注射右美托咪定联合异丙酚,对照组注射等量生理盐水联合异丙酚。记录两组围术期指标、手术前后抑郁自评量表(Self-rating Depression Scale,SDS)、术前及术后血流动力学[心率(heart rate,HR)、平均动脉压(mean arterial pressure,MAP)、脉搏氧饱和度(pulse oxygen saturation,SpO2)]及两组不良反应发生情况。结果 观察组异丙酚用量显著低于对照组(P<0.05)。术后,两组SDS评分均下降(P<0.05),且观察组显著低于对照组(P<0.05)。术后两组HR、MAP较术前时均有显著变化(P<0.05),且观察组HR、MAP显著低于对照组(P<0.05)。观察组术后躁动、注射痛、呼吸抑制、术后头痛发生率显著低于对照组(P<0.05)。结论 右美托咪定联合异丙酚在MECT中能够减轻抑郁,且术中血流动力学更稳定,同时减少不良反应发生。
Abstract
【Abstract】Objective To investigate the application of dexmedetomidine in modified electroconvulsive therapy (MECT) in patients with depression.Methods A total of 106 patients who needed to receive MECT in our hospital from August 2021 to August 2022 were prospectively selected and divided into observation group and control group by random number table method, 53 cases in each group. The observation group was injected with dexmedetomidine combined with propofol, and the control group was injected with the same amount of normal saline combined with propofol. Perioperative indexes, pre-operative and post-operative Self-rating Depression Scale (SDS), preoperative and postoperative hemodynamics [heart rate (HR)] and mean arterial pressure (mean arterial pressure) of both groups were recorded. MAP), pulse oxygen saturation (SpO2)] and the occurrence of adverse reactions in the two groups.Results The dosage of propofol in the observation group was significantly lower than that in the control group (P < 0.05). After operation, SDS scores of both groups were decreased (P < 0.05), and the observation group was significantly lower than the control group (P < 0.05). HR and MAP of the two groups were significantly changed after surgery (P < 0.05), and HR and MAP of the observation group were significantly lower than those of the control group (P < 0.05).The incidence of postoperative agitation, injection pain and respiratory depression in the observation group was significantly lower than that in the control group (P < 0.05). Conclusion Dexmedetomidine combined with propofol in MECT can reduces depression, and the intraoperative hemodynamic stability is more stable, while reducing the occurrence of adverse reactions.
