Analysis of the correlation between clinical symptoms and inflammatory factors in first-episode unmedicated schizophrenia

摘要
探讨分析首发未服药精神分裂症临床症状与炎症因子之间的关系。方法 我们招募了76名参与者,依据《美国精神障碍诊断与统计手册》第五版(DSM-V)诊断标准,分为研究组(首发未服药精神分裂症患者)及对照组(健康人群)。使用超敏电化学发光分析技术(MSD)测定所有人员血清PIGF、VEGF-D、bFGF、IFN-γ、IL-13、IL-1β、IL-2、IL-4、IL-6、IL-8、VEGF、Eotaxin、IP-10、MCP-1水平;同时收集患者相关资料,包括社会人口学资料、阳性和阴性症状量表得分(Positive And Negative Symptom Scale,PANSS)、简明精神病评定量表得分(Brief Psychiatric Rating Scale ,BPRS)及其分量表得分。结果 与健康对照组相比,研究组中的IL-1β、IL-4、bFGF、IL-13、IL-8、IP-10和MCP-1的水平显著升高(P<0.05),VEGF-D的水平低于健康对照(P<0.05)。而IL-2、Eotaxin、PIGF、IFN-γ、IL-6和VEGF在两组间无显著差异(P>0.05)。IL-1β与PANSS量表中阳性因子成正相关(r=0.346,P=0.029);与PANSS量表中焦虑抑郁因子成负相关(r=-0.339,P=0.033),与BPRS量表总分及敌对猜疑因子均成正相关(分别为r=0.384,P=0.015;r=0.365,P=0.02)。MCP-1与PANSS量表中兴奋因子成负相关(r=-0.314,P=0.049)。IL-13与PANSS量表中兴奋因子成负相关(r=-0.326,P=0.04),与BPRS量表中思维障碍因子成负相关(r=-0.34,P=0.032)。通过ROC曲线分析,结果是IL-1β(AUCROC:0.8361,敏感性:85%,特异性:97.22%)。IL-4(AUCROC:0.8469,敏感性:80%,特异性:97.22%)。bFGF(AUCROC:0.9944,敏感性:95%,特异性:97.22%)。IL-8(AUCROC:0.8479,敏感性:90%,特异性:75%)。结论 血清IL-1β、IL-4、bFGF、IL-13、IL-8、IP-10、MCP-1和VEGF-D水平在首发未服药精神分裂症患者及健康人群中存在差异;血清IL-1β、IL-13水平及血清MCP-1水平与部分临床症状存在相关性; 血清IL-1β、IL-4、bFGF及IL-8水平对首发未服药精神分裂症的诊断均有较高价值。
Abstract
[Objective] To investigate and analyze the relationship between clinical symptoms and inflammatory factors in first-episode unmedicated schizophrenia.[Methods] Seventy-six participants were recruited and divided into a study group (patients with first-episode unmedicated schizophrenia) and a control group (healthy population) based on the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V) diagnostic criteria. Serum PIGF, VEGF-D, bFGF, IFN-γ, IL-13, IL-1β, IL-2, IL-4, IL-6, IL-8, VEGF, Eotaxin, IP-10, and MCP-1 levels were measured in all individuals using ultrasensitive electrochemiluminescence analysis (MSD); and relevant patient data, including sociodemographic data, were also collected, Positive And Negative Symptom Scale (PANSS), Brief Psychiatric Rating Scale (BPRS) and its subscales.[Results] The levels of IL-1β, IL-4, bFGF, IL-13, IL-8, IP-10 and MCP-1 were significantly higher (P<0.05) and the levels of VEGF-D were lower (P<0.05) in the study group compared to healthy controls. While IL-2, Eotaxin, PIGF, IFN-γ, IL-6 and VEGF showed no significant difference between the two groups (P>0.05). IL-1β was positively correlated with the positive factor in the PANSS scale (r=0.346, P=0.029); and negatively correlated with the anxiety-depression factor in the PANSS scale (r=-0.339, P=0.033). positively correlated with both the BPRS scale total score and the hostile suspicion factor (r=0.384, P=0.015; r=0.365, P=0.02, respectively). the MCP-1 was negatively correlated with the excitability factor of the PANSS scale (r=-0.314, P=0.049). the IL-13 was negatively correlated with the excitement factor of the PANSS scale (r=-0.326, P=0.04), and with the anxiety and depression factor of the PANSS scale (r=-0.339, P=0.033). P=0.04) and negatively correlated with the thinking disorder factor in the BPRS scale (r=-0.34, P=0.032). By ROC curve analysis, the results were IL-1β (AUCROC: 0.8361, sensitivity: 85%, specificity: 97.22%). IL-4 (AUCROC: 0.8469, sensitivity: 80%, specificity: 97.22%). bFGF (AUCROC: 0.9944, sensitivity: 95%, specificity: 97.22%) . IL-8 (AUCROC: 0.8479, sensitivity: 90%, specificity: 75%).[Conclusion] Serum IL-1β, IL-4, bFGF, IL-13, IL-8, IP-10, MCP-1 and VEGF-D levels differed between patients with first-episode medication-naïve schizophrenia and the healthy population; serum IL-1β, IL-13 and serum MCP-1 levels correlated with some clinical symptoms; serum IL-1β, IL-4, bFGF and IL-8 levels were of high value in the diagnosis of first-episode medication-naïve schizophrenia.
