【摘要】目的:探究重复经颅磁刺激治疗精神分裂症的临床疗效,为临床治疗方案拟定提供依据。方法:研究对象选取为2015.1~2016.12之间我院收治的126例精神分裂症患者,采用数字表法随机分为A、B、C3组各42例,A组患者仅采用利培酮治疗,B组和C组在利培酮治疗基础上进行重复经颅磁刺激治疗,B组患者给予10Hz的伪刺激治疗,C组患者给予10 Hz的真刺激治疗,采用PANSS评价对比各组疗效情况,同时对比治疗前后各组事件相关电位P300和威斯康星卡片分类测验(WCST)情况。结果:A组、B组、C组的总有效率分别为73.8%(31/42)、78.6%(33/42)、95.2%(40/42),C组显著高于A组、B组(P<0.05),但A组与B组之间对比并无显著差异(P<0.05);治疗后A组、B组的P300指标与治疗前对比无显著差异(P>0.05),C组Fz、Cz点的P3、NP3潜伏期较治疗前明显缩短,波幅明显增高(P<0.05);3组患者治疗后WCST中的正确应答、完成分类、概念化水平应答评分较治疗前均显著提高(P<0.05),错误应答、持续性错误、持续性应答数、完成第1个分类所需应答较治疗前均显著降低(P<0.05),且改善水平C组显著高于A组、B组(P<0.05),但A组与B组之间对比并无显著差异(P<0.05)。结论:在利培酮治疗基础上应用10Hz的重复经颅磁刺激可有效改善精神分裂症患者的认知功能,并提高疗效,值得在临床上推广和应用。【关键词】精神分裂症;认知功能;重复经颅磁刺激The effect of repetitive transcranial magnetic stimulation on schizophrenia and its effect on cognitive functionFu chuanchuang,Gan xiaowen,Department of psychiatry; the Seventh Peoples Hospital of Dongguan ,Guangdong 523220Li zhupei,Yuanding,Department of sleep psychology; the Seventh Peoples Hospital of Dongguan;Guangdong; 523220Fan changhe, Department of psychiatry and psychiatry, Guangdong General Hospital (Guangdong Mental Health Institute) 510000[Abstract] Objective: To explore the clinical effect of repetitive transcranial magnetic stimulation in the treatment of schizophrenia, and to provide the basis for clinical treatment. The patients in group A were treated with risperidone alone, Group C and group C were treated with repetitive transcranial magnetic stimulation on the basis of risperidone therapy. Group B patients were given 10Hz pseudo-stimulation therapy. Group C patients were given 10 Hz true stimulation therapy. PANSS was used to evaluate the efficacy of each group. The events related to P300 and Wisconsin Card Classification Test (WCST) were compared before and after treatment. Results: The total effective rates of group A, group B and group C were 73.8% (31/42), 78.6% (33/42) and 95.2% (40/42) respectively, and group C was significantly higher than group A (P <0.05). There was no significant difference between group A and group B (P <0.05). There was no significant difference between group A and group B (P> 0.05) (P <0.05). The correct response, complete classification and conceptual level response score of WCST in the three groups were significantly higher than those before treatment (P <0.05). The positive rate of P3 and NP3 was significantly higher than that of the control group (P <0.05) (P <0.05), and the improvement of the level of C group was significantly higher than the A group, group B (P <0.05), the improvement of the level of C group was significantly higher than the A group, P <0.05), but there was no significant difference between group A and group B (P <0.05). Conclusion: 10Hz repetitive transcranial magnetic stimulation can improve the cognitive function of schizophrenia patients and improve the curative effect on the basis of risperidone therapy. It is worthy to be popularized and applied in clinical practice.
