【摘要】 探究脑卒中后焦虑抑郁合并失眠患者经帕罗西汀治疗后对脑源性神经营养因子、睡眠质量及不良心理状态的影响。方法 选取我科83例脑卒中后焦虑抑郁合并失眠患者,随机分组。对照组41例根据脑卒中病情给予对症治疗同时配合心理治疗,观察组42例增加帕罗西汀治疗,对比两组患者不良情绪,自主神经功能,脑源性神经营养因子,不良反应。结果:观察组焦虑自评该量表(SAS)、匹兹堡睡眠质量指数(PSQI)、交感皮肤反应(SSR)波起始波潜伏期、抑郁自评量表(SDS)评分均低于对照组,神经生长因子(NGF)、SSR波起始波波幅、脑源性神经营养因子(BDNF)、神经营养素-4(NT-4)水平均高于对照组(P<0.05),两组不良反应发生率无统计学意义(P >0.05)。结论:给予脑卒中后焦虑抑郁合并失眠患者心理配合对症治疗联合帕罗西汀治疗后,可有效提升患者脑源性神经因子水平,缓解患者焦虑抑郁情绪,改善睡眠障碍。
Abstract
【Abstract】Objective To explore the brain-derived mental management of patients with anxiety, depression and insomnia after stroke after treatment with paroxetineThe influence of nutritional factors, sleep quality and bad mental state. Methods Eighty-three patients with post-stroke anxiety and depression combined with insomnia in our department were selected and randomly divided into groups. The 41 patients in the control group were given symptomatic treatment and psychotherapy according to the condition of stroke, and 42 patients in the observation group were treated with paroxetine. The adverse mood, autonomic nerve function, brain-derived neurotrophic factor, and adverse reactions of the two groups were compared. Results: The observation groups self-rating anxiety scale (SAS), Pittsburgh Sleep Quality Index (PSQI), sympathetic skin response (SSR) wave initiation period, and self-rating depression scale (SDS) scores were all lower than those of the control group. Growth factor (NGF), SSR wave initial wave amplitude, brain-derived neurotrophic factor (BDNF), neurotrophin-4 (NT-4) levels were higher than those in the control group (P<0.05), and the incidence of adverse reactions in the two groups No statistical significance (P>0.05). Conclusion: After stroke patients with anxiety and depression combined with insomnia combined with symptomatic treatment and paroxetine treatment, they can effectively increase the level of brain-derived neurofactors, alleviate the patients anxiety and depression, and improve sleep disorders.
