Objective To explore the clinical therapeutic effect of inhaled budesonide and terbutaline combined with tiotropium bromide in elderly patients with chronic obstructive pulmonary disease (COPD). Methods A total of 91 elderly patients with COPD admitted from April 2024 to December 2025 were selected and randomly divided into the control group (45 cases) and the observation group (46 cases) according to a random number table. The control group was treated with inhaled budesonide combined with terbutaline, while the observation group was additionally treated with inhaled tiotropium bromide powder on this basis. The intervention effects of the two groups were compared. Results The total effective rate and lung function indices in the observation group were higher than those in the control group (P<0.05); there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Treatment with budesonide and terbutaline combined with tiotropium bromide can improve the total effective rate of treatment, enhance lung function, and has good safety.
