摘要
目的 研究超声剪切波弹性成像技术对肝纤维化患者预后的评估价值,分析其检测指标与临床恢复、实验室检查及生活质量的相关性。方法 选取2025年1月至2026年1月齐齐哈尔市第一医院60例慢性乙型肝炎相关肝纤维化患者,随机分为观察组和对照组各30例;回顾性分析2023年1月至2024年12月30例健康者作为空白对照组。观察组采用2D-SWE联合常规超声检查,其余两组仅行常规超声检查。比较三组的疾病评估价值。结果 观察组杨氏硬度模量数值、ElastPQ数值逐渐升高,CLDQ总分及各维度评分逐渐降低,肝功能指标异常程度加重(P<0.05)。相关性分析显示,杨氏硬度模量数值、ElastPQ数值与CLDQ总分呈显著负相关(r=-0.682、-0.635,P<0.05);AST/PLT比值、FIB-4指数与CLDQ总分呈负相关(r=-0.571、-0.528,P<0.05)。结论 超声剪切波弹性成像技术能评估肝纤维化程度及预后,其检测指标与患者康复和生活质量相关,为无创诊断和预后监测提供依据。
Abstract
Objective To investigate the prognostic value of ultrasonic shear wave elastography (SWE) in patients with liver fibrosis and analyze the correlation between its detection parameters and clinical recovery, laboratory tests, and quality of life. Methods A total of 60 patients with chronic hepatitis B-related liver fibrosis from January 2025 to January 2026 were enrolled at Qiqihar First Hospital and randomly divided into an observation group (n=30) and a control group (n=30). A retrospective analysis was conducted using 30 healthy individuals as a blank control group from January 2023 to December 2024. The observation group underwent 2D-SWE combined with conventional ultrasound examination, while the other two groups received only conventional ultrasound examination. The prognostic value of the three groups was compared. Results In the observation group, the Young's modulus and ElastPQ values gradually increased, while the total score and dimensions of the CLDQ score gradually decreased. The severity of abnormal liver function indicators worsened (P<0.05). Correlation analysis revealed that the Young's modulus and ElastPQ values were significantly negatively correlated with the total CLDQ score (r=-0.682, -0.635, P<0.05), and the AST/PLT ratio and FIB-4 index were negatively correlated with the total CLDQ score (r=-0.571, -0.528, P<0.05). Conclusion Ultrasound shear wave elastography can be used to assess the degree of liver fibrosis and prognosis, and its detection index is related to the recovery and quality of life of patients, which provides a basis for noninvasive diagnosis and prognosis monitoring.
