【摘要】目的 探讨维持性血液透析(Maintenance Hemodialysis,MHD)尿毒症患者抑郁现状并构建风险预测模型。方法 选取2021年3月~2024年3月我院行MHD治疗的尿毒症患者109例为研究对象,按照抑郁情况分为轻度抑郁组(64)和中重度抑郁组(45),比较基线资料、Logistic回归分析危险因素并构建风险预测决策树模型。结果 109例患者抑郁评分平均为(61.56±5.04)分,不同抑郁严重程度尿毒症MHD患者年龄、透析时间、糖尿病、皮肤瘙痒、WBC、CRP、SCr、BUN、TC、TG、ALB、UA、Cys-C、PTH、β2-MG、Hb、P、Ca²⁺、K⁺比较,差异有统计学意义(P<0.05)。Logistic回归分析表明,透析时间是尿毒症MHD患者中重度抑郁的独立危险因素(P<0.05)。性别整体上与抑郁严重程度无显著关联,但在分层分析中,不同性别下其他因素对抑郁的影响不同。对于男性尿毒症MHD患者,皮肤瘙痒是影响抑郁严重程度的重要因素,有皮肤瘙痒的男性患者更易出现中重度抑郁(P<0.05)。对于女性患者,透析时间是关键影响因素,透析时间较长的女性患者中重度抑郁的风险更高(P<0.05)。基于危险因素构建的决策树模型受试者工作特征曲线的曲线下面积为0.778,准确率为67.89%,基于危险因素构建的决策树模型受试者工作特征曲线的曲线下面积为0.778,准确率为67.89%。结论 透析时间是尿毒症MHD患者中重度抑郁的独立危险因素,基于危险因素构建的决策树模型具有较高预测价值,需针对上述机制优化干预策略以改善患者抑郁情况。

【Abstract】Objective Explore the depression status of maintenance hemodialysis (MHD) patients with uremia and construct a risk prediction model.Methods 109 patients with uremia who underwent MHD treatment in our hospital from March 2021 to March 2024 were selected as the research subjects. They were divided into mild depression group (64) and moderate to severe depression group (45) according to their depression status. Baseline data were compared, logistic regression analysis was conducted to identify risk factors, and a risk prediction decision tree model was constructed.Results The mean depression score of 109 patients was (61.56 ± 5.04). The age, dialysis time, diabetes, skin pruritus WBC、CRP、SCr、BUN、TC、TG、ALB、UA、Cys-C、PTH、β2-MG、Hb、P、Ca² The difference between ⁺ and K ⁺ is statistically significant (P<0.05). Logistic regression analysis showed that dialysis time was an independent risk factor for severe depression in uremic MHD patients (P<0.05). A decision tree model based on risk factors has high predictive value, and intervention strategies need to be optimized for the above mechanisms to improve patient prognosis..
