【摘要】目的 探讨五羟色胺受体 2A(5-hydroxytryptamine receptor 2A,HTR2A) rs6313基因表达对奥氮平治疗精神分裂症血药浓度及临床疗效的影响。方法 纳入湖北科技学院附属第二医院2021年1月到2023年6月110例精神分裂症患者,采用实时荧光定量聚合酶链式反应(Polymerase chain reaction,PCR)对HTR2A rs6313基因表达情况进行检测,根据检测结果分为CC型、CT型、TT型组,比较治疗4周、8周时奥氮平血药浓度以及治疗前、后阳性和阴性症状量表(Positive and negative syndrome scale, PANSS)。结果 110例精神分裂症患者HTR2A rs6313基因等位基因中CC型39例,CT型30例,TT型41例;CC型组治疗4周、8周时奥氮平血药浓度均显著高于TT型、CT型组,(P<0.05),CT型显著高于TT型组,(P<0.05);三组治疗前PANSS评分差异无统计学意义(P>0.05),治疗后PANSS评分均显著降低,但CC型显著低于TT型、CT型组,CT型组显著低于TT型组,(P<0.05)。结论 5HTR2A rs6313位点基因突变可能降低奥氮平治疗精神分裂症的血药浓度,影响临床疗效。

| Abstract |
[Abstract] Objective To investigate the effect of 5-hydroxytryptamine receptor 2A (HTR2A) rs6313 gene expression on blood concentration and clinical efficacy of olanzapine in the treatment of schizophrenia. Methods 110 patients with schizophrenia from January 2021 to June 2023 in the Second Hospital Affiliated to Hubei Institute of Science and Technology were included. HTR2A rs6313 gene expression was detected by real-time fluorescence quantitative polymerase chain reaction (PCR). According to the test results, they were categorized into CC-type, CT-type and TT-type groups. The olanzapine blood concentration and the positive and negative syndrome scale (PANSS) before and after treatment were compared at 4 and 8 weeks of treatment. Results Among 110 schizophrenic patients with HTR2A rs6313 gene allele, 39 cases were of CC type, 30 cases were of CT type, and 41 cases were of TT type. olanzapine blood concentration was significantly higher in CC type group than in TT type and CT type group at 4 and 8 weeks of treatment (P<0.05). CT type was significantly higher than TT type group (P<0.05). There was no statistically significant difference in PANSS scores among the three groups before treatment (P>0.05). PANSS scores were significantly lower after treatment, but CC type was significantly lower than TT type and CT type groups, and CT type group was significantly lower than TT type group (P<0.05). Conclusion Mutations at the 5HTR2A rs6313 locus may reduce blood levels of olanzapine in the treatment of schizophrenia and affect clinical efficacy.
