首发精神分裂症患者恐惧表情识别特征的对照研究

摘要
目的:探讨首发精神分裂症患者恐惧表情识别的特征。方法:采用病例对照研究设计,对性别、年龄、文化程度匹配的精神分裂症首次发病患者组(27例)和对照组(28例),应用Ekman标准表情库研究两组受试者对高度恐惧和低度恐惧表情识别的总体应答率、低度恐惧和高度恐惧的准确率和反应时间。结果:病例组的高度恐惧准确率(50.41±6.18)低于对照组(68.29±4.27,P=0.034)。健康对照组低度恐惧的反应时间(362.84±25.23)与准确率(21.79±2.28)呈正相关(r=0.494,P=0.008),病例组阴性症状分(23.35±10.56)与高度恐惧准确率呈负相关(r =-0.428,P=0.042)。结论:首发精神分裂症患者在高度恐惧面孔表情中存在识别障碍,对于恐惧表情识别准确率可以反应患者的阴性症状。
Abstract
Objective: To investigate the characteristics of fear expression recognition in the first-episode schizophrenia patients. Methods: Adopting case-control design, this study using Ekman facial expression of emotion base investigate high and low level fear recognition ability in 27 first-episode schizophrenia patients and 28 healthy controls with matching sex, age and educational level, analyse general response rate of facial expression emotion, correct rate of high and low fear recognition and its response time. Results: The patients’ high level fear recognition accurate rate(50.41±6.18) was lower than that of the control group(68.29±4.27,P=0.034). The response time of the low level fear in the healthy control group(362.84±25.23) was positively correlated with the accuracy rate(21.79±2.28)(r=0.494,P=0.008). The patients’ negative score of PANSS(23.35±10.56) was negatively correlated with the accuracy of high level fear recognition(r =-0.428,P=0.042). Conclusion: First-episode schizophrenia patients have high level fear recognition disorder, which indicates that fear recognition ability reveals the negative symptoms of first-episode schizophrenia patients.
