不同类型精神疾病精神症状与认知功能的差异分析

摘要: 分析抑郁、Ⅰ型、Ⅱ型双相情感障碍和精神分裂症患者的精神症状与认知功能。方法 随机选择2015 年1 月至2016 年1 月在我院治疗的重度抑郁、双相情感障碍和精神分裂症患者各60例，分别为抑郁组、双相组与分裂组，将双相组按分型分为双相Ⅰ型组和双相Ⅱ型组。选取同期60例健康者为对照组，比较各组患者的精神症状、记忆能力与认知功能。结果 双相Ⅰ型组患者的注意/警觉和执行功能评分显著高于双相Ⅱ型组（P<0.05）；分裂组患者的注意/警觉、执行功能、学习记忆和处理速度评分均显著低于抑郁组、双相Ⅰ型组和双相Ⅱ型组（P<0.05）；抑郁组患者的执行功能评分显著低于双相Ⅰ型组，处理速度评分显著高于双相Ⅰ型组（P<0.05）；双相Ⅰ型组患者的YMRS评分显著高于双相Ⅱ型组（P<0.05）。结论 双相Ⅰ型情感障碍、双相Ⅱ型情感障碍、重度抑郁和精神分裂症患者的精神症状与认知功能存在差异，有利于提高确诊率。

| Abstract |
Abstract: Objective To analyze mental symptoms and cognitive function in depression, bipolar disorderⅠ,Ⅱand schizophrenia and to improve the efficiency of diagnosis and treatment. Methods 60 severe depression, 60 bipolar disorder and 60 schizophrenia were randomly selected in January 2015 to January 2016 in our hospital as depression group, bipolar group and the schizophrenia group. The patients of bipolar group were divided into bipolar I group and II group. 60 healthy persons were selected as control group. Mental symptoms, memory ability and cognitive function was compared. Results The score of attention/ alertness and executive function in bipolar I group were significantly higher than those in bipolar II group (P<0.05). The score of attention/ alertness, executive function, learning and memory and processing speed in the schizophrenia group were significantly lower than those in depression group, bipolar I group and bipolar II group (P< 0.05). The score of executive function in depression group was significantly lower than that in bipolar I group, and the processing speed was significantly higher than that in bipolar I group (P<0.05). The score of YMRS in bipolar I group was significantly higher than that in II group (P<0.05). Conclusion There was difference on mental symptoms and cognitive function in the bipolar I, Ⅱ, severe depression and schizophrenia to improve the diagnosis rate.
