摘要	本文旨在基于《伤寒论》“少阳为枢”理论，深入探析“三阳合病，治从少阳”理论的内涵及其在复杂病症中的临床应用策略。研究指出，“三阳合病”多复杂多变，其核心病机在于少阳枢机不利导致表里气机壅塞；“治从少阳”的关键在于抓主证（如口苦、胸胁满），以和解法为枢纽，为解表清里创造内环境；文中以小柴胡汤及其类方（如柴胡加龙骨牡蛎汤）为例，阐释了以和解少阳为核心，根据表、里证偏胜进行灵活化裁的方证对应思路；通过一则老年复杂情志病案分析，印证了该原则在整合经方、处理寒热虚实夹杂病证中的有效性与灵活性。本研究为临床辨治复杂外感及内伤杂病提供了一种以调畅气机为核心的清晰路径与方药运用范式。
Abstract	Based on the theory of “Shaoyang as the pivot” in the Shanghan Lun (Treatise on Cold Damage), this study systematically examines the connotation and clinical application of the principle “treating simultaneous disease of the three yang meridians by focusing on the Shaoyang”. We propose that the complex and variable manifestations of simultaneous three yang disease are rooted in the dysfunction of the Shaoyang pivot, which obstructs qi movement between the exterior and interior. The key to applying this principle lies in identifying cardinal signs (e.g., bitter taste in the mouth, fullness in the chest and hypochondrium) and employing harmonizing methods to restore pivot function, thereby creating an internal environment that facilitates resolution of both exterior and interior pathologies. Using Xiaochaihu Tang (Minor Bupleurum Decoction) and its modified forms, such as Chaihu Jia Longgu Muli Tang (Bupleurum Plus Dragon Bone and Oyster Shell Decoction), as examples, we illustrate a formula syndrome correspondence strategy centered on harmonizing the Shaoyang, with flexible adaptations according to the relative predominance of Taiyang or Yangming patterns. Analysis of a clinical case of a complex emotional disorder in an elderly patient demonstrates the efficacy and adaptability of this approach in integrating classical formulas to manage conditions characterized by intermingled cold, heat, deficiency, and excess patterns. This study offers a clear, qi dynamics oriented therapeutic framework and a practical paradigm for formula application in the management of complex externally and internally originated disorders.
