摘要
目的 探索“医院-家庭-社区”康复护理路径对创伤后手功能障碍患者的应用效果。方法 纳入2024年1月至2025年3月收治的50例创伤后手功能障碍患者,随机分为干预组(n=25)与对照组(n=25)。对照组接受常规康复护理,干预组实施“医院-家庭-社区”三级康复护理路径。比较两组干预前(T0)、出院时(T1)、干预后3个月(T2)的上肢运动功能、腕掌指关节主动活动度、握力功能及日常生活活动能力。结果 干预组在T1及T2时的Fugl-Meyer评分、腕关节背伸活动度、握力及Barthel指数均较干预组有显著改善(P<0.05)。结论 “医院-家庭-社区”康复护理路径可有效促进创伤后手功能障碍患者的手功能恢复,并改善日常生活能力。
Abstract
Objective To explore the application effect of the “hospital-family-community” rehabilitation nursing pathway on patients with post-traumatic hand dysfunction. Methods 50 patients with post-traumatic hand dysfunction admitted from January 2024 to March 2025 were enrolled and randomly divided into intervention group (n=25) and control group (n=25). The control group received conventional rehabilitation nursing, while the intervention group implemented a three-level “hospital-family-community” rehabilitation nursing pathway. Upper limb motor function, active range of motion of the wrist and metacarpophalangeal joints, grip strength, and activities of daily living were compared between the two groups before intervention (T0), at discharge (T1), and 3 months post-intervention (T2). Results The intervention group showed significant improvements in Fugl-Meyer scores, wrist dorsiflexion range of motion, grip strength, and Barthel index at T1 and T2 compared to the control group (P<0.05). Conclusion The “hospital-family-community” rehabilitation nursing pathway can effectively promote hand function recovery and improve activities of daily living in patients with post-traumatic hand dysfunction.
