| 摘要 |
【摘要】探讨早期心理干预对ICU老年患者术后谵妄（POD）及心理健康的影响。方法 将82例ICU老年手术患者，随机分为观察组与对照组，每组41例。对照组给予围手术期常规护理，观察组在对照组基础上给予早期心理护理。对比两组POD发生情况、POD转归，比较两组意识模糊评分量表（DDS）、医院焦虑和抑郁量表（HADA、HADD）评分变化情况。结果 术后72h，观察组的POD发生率为9.76%，显著低于对照组，且POD缓解时间显著短于对照组的31.71%（P<0.05）；麻醉清醒后2h、4h、8h、24h、48h、72h，观察组的DDS评分显著低于对照组（P<0.05），且观察组的POD缓解时间显著短于对照组（P<0.05）；护理后，观察组的HADA和HADD评分显著低于对照组（P<0.05）。结论 早期心理干预可预防或减轻ICU老年患者POD，促进POD康复并改善心理健康状态。

| Abstract |
【Abstract】 Objective To investigate the effect of early psychological intervention on ICU postoperative delirium (POD) and psychological health of elderly patients. Methods A total of 82 elderly patients after surgery in ICU were randomly divided into observation group and control group,with 41 cases in each group. The control group was given routine perioperative nursing, the observation group was given early psychological intervention on the basis of control group.The occurrence and outcome of POD between the two groups were compared,the changes of scores of Delirium Detection Scale (DDS) and Hospital Anxiety and Depression Scale (HADA, HADD) were compared. Results The incidence of POD was 9.76% in the observation group at 72 h after operation, was significantly lower than 31.71% in the control group,and the POD remission time was significantly shorter than the control group (P<0.05);The DDS scores at 2 h, 4 h, 8 h, 24h, 48 h and 72 h after anesthesia awake in the observation group were significantly lower than the control group (P<0.05);After nursing, the scores of HADA and HADD in the observation group were significantly lower than the control group (P<0.05). Conclusion Early psychological intervention can prevent or reduce POD of elderly patients in ICU,which can promote the rehabilitation POD and improve the psychological health.
