团体心理辅导改善康复期精神分裂症患者社会功能的作用

| 摘要 |
验证团体心理辅导对康复期精神分裂症患者社会功能改善作用，帮助患者更好融入社会并为药物治疗结合团体心理治疗的模式提供实践经验。方法 采用病例对照研究，将50例患者随机分到试验组与对照组。对照组维持原有的药物治疗，试验组在原有药物基础上增加每周2次，每次60至90分钟的团体心理辅导。治疗疗程为4周。采用社会功能缺陷筛选量表（Social Disability Screening Schedule ，SDSS）；简明精神病评定量表（BPRS）；社交回避及痛苦量表（SAD）和自我和谐量表（SCCS）四个量表对两组患者进行评估。 结果 1.两组SDSS评分均降低即社会功能缺陷程度减轻，差异显著（P<0.01）；试验组与对照组相比SDSS评分降低更加显著（P<0.01），表现在职业功能、社会退缩、家庭活动、社会活动及生活能力方面。2.BPRS评分比较，治疗后试验组显著低于对照组，症状改善差异显著（t=2.174，P<0.05）。3.两组SAD量表评分，治疗后试验组较对照组社交回避因子分降低更加明显，症状改善更显著（P<0.01）；社交痛苦因子分试验组治疗后比治疗前降低差异显著（P<0.01），试验组治疗效果优于对照组，差异显著（P<0.05）。4.SCCS评分比较，试验组显著低于对照组，两者差异具有统计学意义（t=17.725，P<0.05）。 结论 在药物治疗的基础上联合团体心理辅导，对于康复期精神分裂症患者的社会功能有积极改善作用，是值得推广使用的精神分裂症康复治疗模式。

| Abstract |
Purpose To verify the effect of group psychological counseling on the improvement of social function in patients with schizophrenia during rehabilitation period.In order to help patients better integrate into society and provide practical experience for drug therapy combined with group psychotherapy. Method We adopts Case-control study.We randomly divided 50 patients into test group and control group.The control group maintained the original drug therapy and the test group received group psychological counseling twice a week for 60 to 90 minutes each time which is on the basis of the control group.The course of treatment is 4 weeks.Social Disability Screening Schedule, brief Psychiatric scale, Social avoidance and distress scale (SAD) and Self-Harmony scale (SCCS)were used to study and analyze the two groups of patients. Result 1. The SDSS scores of the two groups were reduced, it means the degree of social dysfunction was reduced, and the difference was significant（P<0.01）.；Compared with the control group, the SDSS score of the test group was significantly lower than that of the control group (P < 0.01), which was manifested in occupational function, social withdrawal, family activities, social activities and living ability.2. The BPRS score was significantly lower in the test group than that in the control group after treatment, and the difference in symptom improvement was significant (P < 0.05).3.Compared with the scores of SAD scale, the scores of social avoidance factors in the test group were significantly lower than those in the control group (P < 0.01), and the symptom improvement was more significant than that in the control group (P < 0.01)；The score of social pain factor in the test group was significantly lower than that before treatment (P < 0.01), and the therapeutic effect in the test group was better than that in the control group (P < 0.05).4. The SCCS score of the test group was lower than that of the control group, and the difference was significant (P < 0.05).. Conclusion On the basis of drug therapy, the combination of group psychological counseling can improve the social function of schizophrenic patients in convalescent period. It is worth popularizing the rehabilitation treatment mode of schizophrenia.
