Abstract Objective To analyze the risk factors of anxiety disorders before senile cataract surgery and multi-mode psychological counseling intervention strategies. Methods The preoperative information of 160 senile cataract patients (160 eyes) treated with phacoemulsification combined with intraocular lens implantation in the ophthalmology department of the hospital from January 2016 to May 2018 were collected to assess the situation of anxiety before surgery. According to the presence of anxiety disorder, the patients were divided into anxiety group (51 cases) and control group (109 cases). Risk factors of anxiety disorder were analyzed, and multi-mode psychological counseling was given to the anxiety group. The improvement of psychological state was evaluated. Results The incidence of anxiety disorder before surgery was 31.88%. There was a statistically significant difference in the score of Hamilton Anxiety Scale (HAMA) between the two groups (P < 0.05). The proportions of patients receiving education for shorter than 12 years, without spouse, family per capita income less than 2,000 yuan/month, no medical insurance, poor sleep quality, no nursing care given by offspring during hospitalization, chronic diseases ≥ 3 kinds, with dyskinesia and vision before surgery < 0.1 in the anxiety group were significantly higher than those in the control group (P<0.05). Logistic regression analysis showed that above-mentioned factors were risk factors for anxiety disorder in elderly cataract patients before surgery (P<0.05). The HAMA score, SAS score and SDS score in the anxiety group decreased significantly after psychological counseling (P<0.05). Conclusion The risk factors of anxiety disorder in elderly cataract patients before surgery include education level, family income, medical insurance, sleep quality, types of chronic diseases,dyskinesia and vision before surgery, etc.. Carrying out multi-mode psychological intervention before surgery can prevent the incidence of anxiety disorder before surgery.
