前扣带回皮质喙部(rostral anterior cingulate cortex, rACC)主要负责情绪性冲突的监控,也是抑郁症伴自杀意念(major depression disorder with suicidal ideation, SI)患者情感环路的关键节点。既往研究已经证实与前扣带回皮质喙部相连的皮层环路的静态信息整合异常。然而,人类大脑功能网络更多表现为时变的动态特性,前扣带回皮质喙部环路信息时变整合仍然不清楚。方法 本研究收集了28例抑郁症伴自杀意念患者的静息状态的功能磁共振数据,并收集30名年龄、性别和教育年限匹配的健康对照。采用以前扣带回皮质喙部为种子点的时间滑窗的功能连接方法,比较前扣带回皮质喙部环路信息时变整合异常表征。探测抑郁症伴自杀意念患者大脑rACC环路动态功能连接的变异特征。结果 较健康,患者的左侧前顶叶、颞中回、颞上回、内侧额上回、缘上回和右侧中央沟盖、内侧额上回、颞中回、中央后回与喙部前扣带回的动态功能连接显著增加。进一步,发现自杀意念分数分别与右侧中央沟盖(r = 0.4869, P = 0.0086)、左侧颞中回和左侧颞上回(r = 0.5714, P = 0.0015)的动态功能连接值呈正相关。结论 本研究表明抑郁症伴自杀意念患者的前扣带回皮质喙部环路的信息时变整合的变异性增强,而稳定性降低,可能会影响情绪监控和决策。该项研究为抑郁症的病理生理机制提供了影像学证据。

| Abstract |
Objective The rostral anterior cingulate cortex (rACC), which is mainly involved in the monitoring of emotional conflicts, is also the critical hub of affective circuits in major depression disorder patients with suicidal ideation (SI). Previous studies have confirmed that the static information integration in the rostral anterior cingulate cortical pathways was abnormal in patients. However, in most cases, human brain functional networks exhibit time-varying dynamic characteristic. The time-varying information integration in the rostral anterior cingulate cortical circuits remains unclear in major depression disorder patients with SI.Methods This study recruited 28 major depression disorder patients with SI and 30 age, sex and education level matched healthy controls to scan resting-state functional magnetic resonance imaging. The previous reported rACC was selected as seed region to compute dynamic functional connectivity using sliding windows. Group comparisons were carried out to found the abnormal time-varying information integration patterns of the rostral anterior cingulate cortical circuits in patients. Finally, the study could characterize patients dynamic functional connectivity of the rACC pathway aberrance.Results Compared with healthy controls, patients exhibited significantly increased dynamic functional connectivity in the left anterior parietal cortex, middle temporal gyrus, superior temporal gyrus, medial superior frontal gyrus, superior marginal gyrus and right central sulcus, medial superior frontal gyrus, middle temporal gyrus, postcentral gyrus and rostral anterior cingulate gyrus. Furthermore, the SI scores positively correlated with the dynamic functional connectivity of the right central sulcus (r = 0.4869, P = 0.0086), left middle temporal gyrus and superior temporal gyrus (r = 0.5714, P = 0.0015) in patients.Conclusions This study suggested that the time-varying information integration of the rACC circuits in major depression disorder patients with SI exhibited increased variability and reduced stability, which may influence the emotional monitoring and decision making. The study added the neuroimaging evidences for the biological mechanism of depression disorder.
