目的:探讨双相抑郁患者腺苷酸环化酶-2(ADCY-2)基因多态性与认知功能及改良电休克治疗(MECT)疗效关系。方法:选取2016年1月~2017年12月在我院诊治的双相抑郁患者80例作为研究对象,采用TaqMan基因分型技术检测ADCY-2基因rs2892609、rs1864071单核苷酸多态性的分布,采用事件相关电位检测(P300)和威斯康辛卡片分类测验(WSCT)评估双相障碍患者的认知功能,对患者进行8次MECT治疗,以汉密尔顿抑郁量表(HAMD)总分评估MECT疗效,分析ADCY-2基因多态性与认知功能及MECT疗效关系。结果:ADCY-2基因rs2892609位点不同基因型CC、PE、RE、潜伏期评分比较,rs1864071位点不同基因型TT、CC、PE、RE、潜伏期、波幅评分比较,差异显著(P<0.05);rs2892609位点GG型的CC评分明显高于GT型、TT型,RE、潜伏期评分明显低于GT型、TT型,rs1864071位点AA型的PE评分明显低于AG型、GG型,波幅评分明显高于AG型、GG型,差异显著(P<0.05)。ADCY-2基因rs2892609位点GG型与GT型、rs1864071位点AA型与AG型治疗前、第8次治疗HAMD评分比较,差异不显著(P﹥0.05);rs2892609位点GG型第2、4、6次治疗HAMD评分明显高于GT型,rs1864071位点AA型第2、4、6次治疗HAMD评分明显高于AG型,差异显著(P<0.05)。结论:双相抑郁患者ADCY-2基因rs2892609、rs1864071位点的基因多态性与认识功能障碍有关,rs2892609位点的GT基因型、rs1864071位点的AG基因型对MECT治疗的起效更快,反应性更好。

| Abstract |
Objective: To explore the relationship between adenylate cyclase-2 (ADCY-2) gene polymorphism and cognitive function and the effect of improved electrical shock therapy (MECT) in patients with bipolar depression. Methods: 80 patients with bipolar depression diagnosed and treated in our hospital from January 2016 to December 2017 were selected as the research subjects. TaqMan genotyping technology was used to detect the distribution of ADCY-2 genes rs2892609 and rs1864071 single nucleotide polymorphisms. The event-related potential P300 and the Wisconsin Card Classification Test (WSCT) were used to evaluate the cognitive function of patients with bipolar disorder. The patients were treated with 8 MECTs. The total score of the Hamilton Depression Scale (HAMD) was used to evaluate the efficacy of MECT. The ADCY was analyzed. -2 gene polymorphism and cognitive function and MECT efficacy. Results: Comparison of CCY, PE, RE, and latency scores of different genotypes of ADCY-2 gene rs2892609 locus. Comparison of TT, CC, PE, RE, latency, and volatility scores of different genotypes of rs1864071 locus showed significant differences (P <0.05); rs2892609 The CC score of GG type is significantly higher than that of GT type and TT type. The RE and latency scores are significantly lower than those of GT type and TT type. The PE score of AA type at rs1864071 is significantly lower than that of AG type and GG type, and the amplitude score is significantly higher. AG type and GG type were significantly different (P <0.05). ADCY-2 gene rs2892609 locus GG type and GT type, rs1864071 locus AA type and AG type before and after the 8th treatment of HAMD scores, the difference was not significant (P) 0.05); The HAMD scores of 6 and 6 treatments were significantly higher than those of GT type, and the HAMD scores of rs1864071 loci at 2nd, 4th and 6th treatments were significantly higher than those of AG type, the difference was significant (P <0.05). Conclusion: The polymorphisms of rs2892609 and rs1864071 loci of ADCY-2 gene in patients with bipolar depression are related to cognitive dysfunction. The GT genotype of rs2892609 locus and the AG genotype of rs1864071 locus have a faster effect on MECT therapy. Better reactivity.
