| 摘要 |
目的:探讨扩散张量成像(diffusion tensor imaging,DTI)结果预测早期卒中后认知障碍(post stroke cognitive impairment, PSCI)的应用价值。方法:回顾性纳入2018年12月至2019年12月间我院收治的62例卒中早期PSCI患者(PSCI组),并招募同期60例健康体检者作对照组。主要观察指标为DTI检查结果,蒙特利尔认知量表(MoCA)评估的认知功能评分。对比两组DTI结果,将各项异性分数(FA)和MoCA评分进行相关性分析,绘制ROC曲线评估DTI对PSCI的预测效能。结果:PSCI组两侧额叶、两侧海马、胼胝体FA均低于对照组(P<0.05);两侧额叶(左侧:r=0.499,P=0.000;右侧:r=0.395,P=0.000)、两侧海马(左侧:r=0.355,P=0.000;右侧:r=0.506,P=0.000)、胼胝体FA值(r=0.409,P=0.000)均与MoCA评分呈正相关;右侧海马FA预测早期PSCI的最佳截点为0.22,此时AUC为0.911,显著优于左侧海马FA、右侧额叶FA、胼胝体FA(Z=2.488,2.039,2.936;P=0.013,0.041,0.003),右侧海马FA、左侧海马FA、左侧额叶FA诊断敏感性分别为93.50%、85.52%、83.92%,显著优于其他指标(P<0.05)。结论:DTI可显示早期PSCI患者脑结构网络存在的连接异常,两侧额叶、两侧海马、胼胝体脑白质区域FA值改变对PSCI发生具有良好的预测效果。

| Abstract |
Objective To explore application value of diffusion tensor imaging(DTI)result for predicting early post stroke cognitive impairment(PSCI).Methods Retrospective analysis were performed on 62 early stroke combined with PSCI patients(PSCI group)treated in our hospital from December 2018 to December 2019,and 60 healthy controls were employed as control group.DTI results and cognitive functions scores by Montreal Cognitive Assessment(MoCA)were primary observed indicators.Two groups’ DTI results were comparative analysed,analysing the correlation between fractional anisotropy(FA)and MoCA scores,constructing ROC curve to assess prediction efficiency of DTI for predicting PSCI.Result Bilateral frontal lobe FA,bilateral hippocampus FA,callosum FA in PSCI group were lower than those in the control group(P<0.05);Bilateral frontal lobe FA(left:r=0.499,P=0.000;right:r=0.395,P=0.000),bilateral hippocampus FA(left:r=0.355,P=0.000;right:r=0.506,P=0.000),callosum FA(r=0.409,P=0.000)were positive correlation with MoCA scores;Cut-off of right hippocampus FA to predict early PSCI was 0.22,the AUC was 0.911 and higher than that of left hippocampus,right frontal lobe,callosum(Z=2.488,2.039,2.936;P=0.013,0.041,0.003),the sensitivity of right hippocampus FA,left hippocampus FA,left frontal lobe FA were 93.50%,85.52%,83.92% and were better than other indicators(P<0.05).Conclusion DTI can show the abnormal connection in brain networks in early PSCI patients,FA variation on brain white matter of bilateral frontal lobe,bilateral hippocampus and callosum have a good predictive performance for PSCI.
