摘要:探讨睡眠质量和负性情绪对绝经后妇女认知功能的影响。方法 选择80例绝经后妇女作为研究对象,采用简易智能筛查量表(MMSE)评估认知功能,并分为认知降低组与认知正常组。采用匹兹堡睡眠质量指数(PSQI)评估睡眠质量,采用抑郁自评量表(SDS)评价抑郁状态,采用焦虑自评量表(SAS)评价焦虑状态。所有受试者均空腹抽血检测雌二醇(E2) 、睾酮(T)等卵巢激素水平。结果 认知降低组睡眠障碍比例63.6%明显高于认知正常组34.5%(P<0.05);认知降低组PSQI评分高于认知正常组(P<0.05)。两组受试者中存在睡眠障碍者SAS及SDS评分均高于睡眠正常者(P<0.05);认知降低组睡眠障碍者及睡眠正常者SAS及SDS评分均高于认知正常组(P<0.05)。两组受试者中睡眠障碍者E2水平低于睡眠正常者(P<0.05);认知降低组睡眠障碍者及睡眠正常者E2水平均低于认知正常组(P<0.05)。绝经后妇女MMSE与PSQI、SDS及SAS呈负相关关系(P<0.05),与E2水平呈正相关关系(P<0.05) ;PSQI与SDS及SAS呈正相关关系(P<0.05) ,与E2水平呈负相关关系(P<0.05) ;SDS与SAS呈正相关关系(P<0.05) ,与E2水平呈负相关关系(P<0.05) ;SAS与E2水平呈负相关关系(P<0.05)。结论 绝经后女性睡眠障碍及负性情绪可以影响其认知功能,因此对于该类人群需引起特别重视,E2水平降低可能是三者之间共同的机制之一,可以作为临床研究方向。
Abstract
Abstract: Objective To explore the effects of sleep quality and negative emotions on cognitive function of postmenopausal women. Methods Eighty post-menopausal women were selected as the research object, and the simple intelligent screening scale (MMSE) was used to evaluate cognitive function, and they were divided into cognitive decline group and normal cognitive group. The Pittsburgh Sleep Quality Index (PSQI) was used to evaluate sleep quality, the Depression Self-rating Scale (SDS) was used to evaluate depression status, and the Anxiety Self-rating Scale (SAS) was used to evaluate anxiety status. All subjects were drawn with fasting blood to detect ovarian hormone levels such as estradiol (E2) and testosterone (T). Results The proportion of sleep disturbance in the cognitive reduction group was 63.6%, which was significantly higher than that in the normal cognitive group (34.5%) (P <0.05); the PSQI score in the cognitive reduction group was higher than that in the normal cognitive group (P <0.05). The SAS and SDS scores of those with sleep disorders in both groups were higher than those of normal sleep (P <0.05); the SAS and SDS scores of sleep disorders and normal sleep in cognitive decline group were higher than that of normal cognitive group (P <0.05). The E2 level of sleep-disordered persons in the two groups was lower than that of normal sleepers (P <0.05); the E2 level of sleep-disordered and normal sleepers in the cognitively reduced group was lower than that in the cognitively normal group (P <0.05). Postmenopausal womens MMSE was negatively correlated with PSQI, SDS and SAS (P <0.05), and positively correlated with E2 levels (P <0.05); PSQI was positively correlated with SDS and SAS (P <0.05), was correlated with E2 levels Negative correlation (P <0.05); SDS was positively correlated with SAS (P <0.05) and negatively correlated with E2 level (P <0.05); SAS was negatively correlated with E2 level (P <0.05). Conclusion Postmenopausal womens sleep disturbances and negative emotions can affect their cognitive function, so special attention should be paid to this group of people. The reduction of E2 level may be one of the common mechanisms among the three and can be used as a clinical research direction.
