【摘要】目的:研究观察抑郁症患者无抽搐电休克治疗(MECT)分别使用右美托咪定、氯胺酮麻醉时对眼压(intraocular pressure,IOP)及抑郁减分的影响。方法:选择我院收治的300例抑郁症患者,病例来源2017年2月至2018年12月。根据随机数表法进行分组,设为对照组与观察组,每一组150例。两组患者均接受MECT治疗,其中对照组麻醉时选择使用氯胺酮,观察组麻醉时选择右美托咪定。对比观察两组患者的眼压、抑郁减分、治疗前后血清标志物以及并发症情况。结果:(1)T0与T2时,两组患者的眼压无明显诧异(P>0.05),T1时,观察组眼压水平稍微高于对照组,存在统计学意义(P<0.05)。(2)T2时,观察组抑郁症综合评分低于对照组,差异显著,有统计学意义(P<0.05)。(3)治疗后,观察组IL-2、IL-6水平低于对照组,两组数据分析,差异明显,存在统计学意义(P<0.05)。(4)统计显示,观察组4例发生并发症,对照组14例,比较发生率,观察组低于对照组(P<0.05)。结论:相比氯胺酮,右美托咪定在抑郁症患者无抽搐电休克治疗中的作用更显著,有助于缓解抑郁情绪,改善血清标志物,并发症少,但需要注意对眼压的影响。

| Abstract |
[Abstract] Objective: To study the effects of dexmedetomidine and ketamine anesthesia on intraocular pressure (IOP) and depression score in patients with depression. Methods: 300 patients with depression were selected from February 2017 to December 2018. They were divided into control group and observation group, 150 cases in each group. The patients in both groups were treated with MECT. Ketamine was used in the control group and dexmedetomidine was used in the observation group. The intraocular pressure, depression score, serum markers before and after treatment and complications were compared between the two groups. Results: (1) at t0 and T2, there was no significant surprise in intraocular pressure (P > 0.05) zero point zero five )At T1, the intraocular pressure level in the observation group was slightly higher than that in the control group, with statistical significance (P< zero point zero five )。 (2) At T2, the comprehensive score of depression in the observation group was significantly lower than that in the control group (P< zero point zero five )。 (3) After treatment, the levels of IL-2 and IL-6 in the observation group were lower than those in the control group, and there was significant difference between the two groups (P< zero point zero five )。 (4) The results showed that there were 4 complications in the observation group and 14 in the control group. The incidence of complications in the observation group was lower than that in the control group (P< zero point zero five )。 Conclusion: compared with ketamine, dexmedetomidine plays a more significant role in the treatment of non convulsive electroconvulsive in patients with depression. It is helpful to relieve depression, improve serum markers, and have fewer complications. However, we need to pay attention to the effect on intraocular pressure.
