摘要
【目的】研究稳定期精神分裂症患者神经认知功能损害特征及其影响因素,为针对性改善精神分裂症患者神经认知功能提供临床应对策略。【方法】155名稳定期精神分裂症患者及36名健康受试者接受了MATRICS共识认知成套测试(MCCB)以及简明精神病评定量表(BPRS)的评估,收集受试者的一般人口学信息及临床资料,比较精神分裂症患者与健康受试者之间神经认知功能的差异,阐明影响精神分裂症患者神经认知功能的相关因素。【结果】(1)稳定期精神分裂症患者六个神经认知维度得分均显著低于健康对照组(P<0.05)。(2)回归分析显示:精神病理症状严重程度(BPRS总分)对精神分裂症患者信息处理速度和言语学习功能影响显著(P<0.05);(3)精神分裂症患者的六个神经认知维度之间有显著关联(P<0.05):信息处理速度显著影响注意/警觉性、工作记忆、言语学习、推理和问题解决能力(P<0.01);工作记忆显著影响注意/警觉性、信息处理速度、视觉学习(P<0.01);注意/警觉性对工作记忆和信息处理速度影响显著(P<0.01)。【结论】(1)与健康对照组比较,稳定期精神分裂症患者六个神经认知维度的功能均显著受损;(2)精神病理症状严重程度显著影响精神分裂症患者的信息处理速度和言语学习功能;(3)精神分裂症患者的六个神经认知维度之间互相影响,信息处理速度下降可能是患者核心神经认知功能障碍,广泛影响其它神经认知功能。
Abstract
Objective To explore the characteristics of neurocognition impairment in patients with stable schizophrenia, and to analyze related factors, and to provide clinical coping strategies for improving and enhancing neurocognitive function in patients with schizophrenia. Methods 155 patients with stable schizophrenia and 36 healthy subjects were assessed by the MATRICS Consensus Cognitive Cognitive Test (MCCB) and the Brief Psychiatric Rating Scale (BPRS). General demographic and clinical data were collected,comparing differences in cognitive function between patients with schizophrenia and healthy controls, and to explore related factors that affect neurocognitive function in patients with schizophrenia. Results (1) The scores of the six neurocognitive dimensions in patients with stable schizophrenia were significantly lower than those in the healthy control group (P<0.05). (2) Regression analysis showed that the severity of psychopathological symptoms (total BPRS score) had significant effects on information processing speed, verbal learning in patients with schizophrenia (P<0.05); (3) There are significant correlations between the six neurocognitive dimensions of schizophrenia (P<0.05):the speed of information processing significantly affects attention/alertness, working memory, verbal learning, reasoning and problem solving ability (P<0.01); Working memory significantly affects attention/alertness, information processing speed, and visual learning (P<0.01); attention/alertness significantly affects working memory, information processing speed, and social cognition (P<0.01). Conclusions (1) Compared with the healthy control group, the functions of the six neurocognitive dimensions of patients with stable schizophrenia were significantly impaired; (2) The severity of psychopathological symptoms significantly affected the information processing speed, verbal learning and social cognitive functions of patients with schizophrenia; (3) The six neurocognitive dimensions of patients with schizophrenia are related to each other, the decrease of information processing speed may be the core neurocognitive dysfunction of patients, which widely affects other neurocognitive functions.
