Objective To explore the causes of dispensing errors in Western medicine pharmacies and preventive measures. Methods Collect 870 prescriptions from January 2022 to December 2024, and statistically analyze the number and types of errors in these prescriptions to delve into the factors that lead to dispensing errors. Results Dispensing errors in Western medicine pharmacies are not limited to one type; common ones include category errors, dosage errors, incorrect recipient errors, contraindication errors, and quantity errors. Factors such as high similarity in drug names, similar packaging, multiple specifications of the same drug, and insufficient understanding of drug contraindications by relevant personnel are all major contributors to dispensing errors. Conclusion Dispensing error incidents in Western medicine pharmacies exhibit diverse types and complex causes. Therefore, it is essential to place great emphasis on pharmacy management, enhance the professional capabilities of pharmacists, encourage them to develop a conscientious and responsible work attitude, improve prescription review systems, optimize the quality of prescription dispensing comprehensively, minimize dispensing errors in clinical settings, and ensure patient safety in medication use.
