【摘要】 分析首次胰岛素治疗的2型糖尿病患者给予自我实践为主导的施教法联合心理干预后患者心理状态及血糖水平变化。方法 将我院200例首次胰岛素治疗的2型糖尿病患者依据掷币法分为心理干预组100例及联合施教组100例,两组患者均给予健康教育及注射演示,在此基础上心理干预组增加心理干预治疗,联合施教组在心理干预组基础上增加自我实践为主导的施教法进行干预,对比干预后两组患者胰岛素注射情况、血糖水平及心理状况改变。结果 干预后,联合施教组SAS、SDS评分、空腹血糖、餐后2h血糖、糖化血红蛋白水平、注射部位不良反应发生率低于心理干预组,胰岛素注射轮换部位率、注射规范率、注射技能评分高于心理干预组(P<0.05);两组针头一次性使用率无统计学意义(P>0.05)。结论 自我实践为主导的施教法联合心理干预能够有效提升2型糖尿病首次胰岛素治疗患者的注射技能,缓解焦虑抑郁情绪,控制血糖水平。

| Abstract |
[Abstract] Objective To analyze the changes in the mental state and blood glucose levels of patients with type 2 diabetes who were treated with insulin for the first time after they were given self-practice-led teaching method combined with psychological intervention. Methods According to the coin toss method, 200 patients with type 2 diabetes who were treated with insulin for the first time were divided into psychological intervention group of 100 cases and joint teaching group of 100 cases. Patients in both groups were given health education and injection demonstration, based on psychological intervention Psychological intervention treatment was added to the combined teaching group, and the self-practice-led teaching method was added to the psychological intervention group to intervene in the combined teaching group. After the intervention, the insulin injection, blood sugar level and psychological status of the two groups were compared. Results After the intervention, the combined education groups SAS, SDS score, fasting blood glucose, 2h postprandial blood glucose, glycosylated hemoglobin level, and the incidence of adverse reactions at the injection site were lower than those in the psychological intervention group. The insulin injection rotation site rate, injection standard rate, and injection skill score It was higher than that of the psychological intervention group (P<0.05); the disposable needle use rate in the two groups was not statistically significant (P>0.05). Conclusion Self-practice-led teaching method combined with psychological intervention can effectively improve the injection skills of patients with type 2 diabetes who are treated with insulin for the first time, relieve anxiety and depression, and control blood sugar levels.
