摘要
【摘要】 研究不同强度神经肌肉电刺激对小儿脑瘫吞咽障碍和精神状况的影响。 方法 选择2017年1月~2019年1月本院80例小儿脑瘫吞咽障碍患儿作为研究对象,用随机数表法分为观察组和对照组,两组各40例。两组均采用口腔感觉刺激和神经肌肉电刺激治疗,观察组刺激强度为20mA,对照组刺激强度为10mA。对比分析两组患儿治疗前后临床疗效、吞咽障碍调查(DDS)评分、简易精神状态评价量表(MMSE)评分、汉密尔顿抑郁量表(HAMD)评分。结果 治疗后,观察组总有效率明显高于对照组,差异有统计学意义(P<0.05);治疗后,观察组DDS腔期、咽期、食管期评分均低于对照组,差异有统计学意义(P<0.05);治疗后4周、8周,观察组MMSE评分均高于对照组,差异有统计学意义(P<0.05);治疗后4周、8周,观察组HAMD评分均低于对照组,差异有统计学意义(P<0.05)。结论 使用神经肌肉电刺激治疗小儿脑瘫吞咽障碍时,在患儿耐受的范围内,电刺激强度越高,吞咽障碍和精神状况改善效果越好。
Abstract
【Abstract】 Objective To study the intervention effect of different intensities of neuromuscular electrical stimulation on dysphagia in children with cerebral palsy.Both groups were treated with oral sensory stimulation and neuromuscular electrical stimulation. The stimulation intensity of the observation group was 20mA, while that of the control group was 10mA.Methods 80 children with dysphagia due to cerebral palsy in our hospital from January 2017 to January 2019 were selected and randomly divided into observation group and control group with 40 cases in each group.The clinical efficacy, dysphagia Survey (DDS) score,Mini mental state assessment (MMSE) score, Hamilton Depression Scale (HAMD) score were compared between the two groups before and after treatment.Results After treatment, the total effective rate of observation group was significantly higher than that of the control group, the difference was statistically significant (P<0.05);After treatment, the scores of DDS in the observation group were lower than those in the control group (P<0.05);After 4 weeks and 8 weeks of treatment, the MMSE score of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05); After 4 weeks and 8 weeks of treatment, the HAMD score of the observation group was lower than that of the control group, the difference was statistically significant (P<0.05).Conclusion When neuromuscular electrical stimulation is used to treat dysphagia in children with cerebral palsy, the higher the intensity of electrical stimulation is, the better the improvement of dysphagia and mental status
