Community specialized nursing clinics are specialized nursing clinics established by community health service centers to meet the specific health needs of residents. On the basis of providing basic nursing services, community specialized nursing clinics also provide professional nursing services for specific health problems of residents, such as diabetes, hand injury dressing change, vascular access maintenance, etc. The case management model is a comprehensive nursing model dominated by nurses, covering multiple disciplines, departments, and specialties. Its main purpose is to provide patients with specialized, systematic, holistic, and individualized nursing services, eliminate nursing risks, improve clinical efficacy, and accelerate early recovery. Based on this, this article first introduces the concept of case management, the operation process of case management, and the common modes of case management, and summarizes the application progress of case management in community specialized nursing clinics, in order to provide reference for relevant personnel and nursing work.
