摘要
双相障碍(Bipolar disorder,BD)是一类常见的精神障碍,其发病机制与病理生理改变尚不明确。近年来有证据表明神经免疫系统可能参与了BD的病理过程,BD涉及免疫细胞的激活和中枢神经系统(Central nervous system,CNS)炎症物质的释放, 但其内在联系尚不完全清楚。BD患者外周血的C-反应蛋白(C-Reactive protein,CRP)以及促炎细胞因子(如:IL-1,IFN-γ,IL-6,TNF-α)和抑炎细胞因子(如:IL-4、IL-10、TGF-β)水平存在异常,但在不同性别、情感状态和疾病阶段下相关标志物水平具有明显差异。另外体重、合并症以及药物治疗也会影响到相关标志物的水平。明确神经免疫在BD的发病机制中的作用对BD的临床诊断和预后以及预测药物疗效具有潜在价值。
Abstract
Bipolar disorder is a common type of mental disorder, and its pathogenesis and pathophysiological changes are still unclear. In recent years, there is evidence that the neuroimmune system may be involved in the pathological process of BD. BD is involved in the activation of immune cells and the release of inflammatory substances in the central nervous system, but its internal connection is not completely clear. The levels of C-reactive protein, pro-inflammatory cytokines(such as IL-1,IFN-γ,IL-6,TNF-α) and anti-inflammatory cytokines(such as IL-4、IL-10、TGF-β) in the peripheral blood of BD patients are abnormal, and the levels are significantly different in different genders, emotional states and disease stages. In addition, weight, comorbidities and drug treatment will also affect the level of relevant markers. Clarifying the role of neuroimmunity in the pathogenesis of BD has potential value for the clinical diagnosis, prognosis and predicting drug efficacy of BD.
