轻度精神发育迟滞患者性自我防卫能力鉴定结果的一致性和变化研究：一项Meta分析

研究近30年来轻度精神发育迟滞患者性自我防卫能力鉴定结果的一致性和变化。方法 从中国期刊全文数据库、万方数据库、维普论文检测系统、PubMed 和Cochrane Library进行文献检索，搜索关于性自我防卫能力鉴定相关文献，由2位评价员独立筛选文献、提取资料，釆用 CMA 2.0 软件进行 Meta 分析。结果 共纳入17篇文献，421例轻度精神发育迟滞患者，其中41例评定为有性自我防卫能力，比率为12.1% （95%CI：7.4%-19.1%）；262例评定为性自我防卫能力削弱，比率为59.4% （95%CI：46.6%-71.0%）；118例评定为无性自我防卫能力，比率为30.4% （95%CI：20.7%-42.3%）。亚组分析显示，性防卫能力评定结果与是否为省会城市无关。Meta回归分析显示轻度精神发育迟滞患者评定为有性自我防卫能力的比率随着发表年限的增加而逐渐降低[Slope=-0.05，95%CI（-0.09，-0.006），P<0.05]。结论 轻度精神发育迟滞患者性自我防卫能力鉴定结果差异较大，评定为性自我防卫能力削弱的比率最高，其次为无性自我防卫能力，而评定为有性自我防卫能力的比率逐渐降低。这为司法鉴定工作提供了一定指导，也提示司法鉴定的一致性培训工作但仍需继续开展。

| Abstract |
Objective To estimate the incidence of sexual defense capacity among mild mental retardation by meta-analysis and to provide advice for the evaluation of sexual defense capacity among mild mental retardation. Methods We searched databases including CNKI, WanFang Data, VIP, PubMed, The Cochrane Library, to collect literature regarding the incidence of sexual defense capacity among mild mental retardation. Two reviewers independently screened literature, extracted data. Then meta-analysis was performed by using Comprehensive Meta Analysis(CMA) 2.0 software. Results 17 research were included, containing 421 mental retardation, of which 41 were evaluated having sexual defense capacity, the ratio was 12.1% [95%CI (7.4%，19.1%); 262 were evaluated having partial sexual defense capacity, the ratio was 59.4% [95%CI (46.6%，71.0%); 118 were evaluated no sexual defense capacity, the ratio was 30.4% [95%CI (20.7%，42.3%). There was no statistical differences observed when the studies were stratified by regions, diagnostic criteria (all P values>0.05). Meta regression analysis showed that the publication year is negative correlated with the ratio of having sexual defense capacity. Conclusion The highest ratio of sexual defense capacity among mild mental retardation is partial-capacity, then no-capacity. The result of evaluating having sexual defense capacity is lower and lower years by years.
