经颅直流电刺激对卒中后抑郁治疗效果的Meta分析

摘要	摘要：系统评价经颅直流电刺激（transcranial direct current stimulation, tDCS）对卒中后抑郁（post-stroke depression, PSD）的治疗效果。方法检索Cochrane Library、PubMed、Web of Science、中国知网、万方数据库及维普中文科技期刊数据库中关于tDCS对PSD治疗的临床随机对照试验（randomized controlled trial, RCT），检索时限从数据库建立至2019年8月。采用Cochrane协作网中RCT偏倚风险评价标准进行方法学质量评估，采用Review Manager 5.3软件进行Meta分析。结果共纳入6项研究。与对照组相比，tDCS能够明显改善卒中后患者抑郁情况（SMD=1.85,95%CL=1.13~2.57,P<0.00001），可见蒙哥马利抑郁量表（Montgomery Depression Scale, MADRS）、24项汉密尔顿抑郁量表（和Hamilton depression scale, HAMD）、汉密尔顿焦虑量表（Hamilton Anxiety Scale, HAMA）、抑郁自评量表（Self-rating depression scale, SDS）、贝克抑郁测量表（The Beck Depression Inventory，BDI）评分具有显著性差异（P<0.05），17项汉密尔顿抑郁量表（Hamilton Depression Rating Scale, HDRS）评分无统计学意义（P>0.05），其余临床结局采用描述性分析。
Abstract	Abstract：Objective to systematically evaluate the therapeutic effect of transcranial direct current stimulation on post-stroke depression. Methods The randomized controlled trials, retrieval time of clinical randomized controlled trial of tDCS for post-stroke depression was searched in Cochrane Library、PubMed、Web of Science、CNKI, Wanfang database and Weipu database from database establishment to August 2019. The RCT bias risk evaluation standard in Cochrane collaborative network is used to evaluate the methodological quality, and Review Manager 5.3 software is used for Meta analysis. Results A total of 6 studies were included. Compared with the control group, tDCS could significantly improve the depression after stroke (SMD=1.85,95% CL1.13~2.57, P<0.00001), and the MADRS, HAMD, HAMA, SDS, BDI score was significantly different(P<0.00001). HDRS score was not statistically significant(P>0.05),The other clinical outcomes were analyzed by descriptive analysis. Conclusion tDCS can improve the depression of PSD patients. Due to the limitation of quality and quantity, the above conclusions still need to be verified by more high-quality studies.
