【摘要】目的：探究超声引导下髂筋膜间隙阻滞(FICB)对老年髋部骨折患者术后认知功能、应激相关激素及中枢神经特异蛋白(S-100β)水平的影响。方法：选取我科2018年3月至2020年9月期间121例老年髋部骨折患者作为研究对象，采用抽签法将其分组。对照组60例术后给予静脉注射氟比洛芬酯镇痛，观察组61例术后给予超声引导下FICB。对比两组患者认知功能、应激相关激素、不良反应发生情况。结果：观察组患者简易精神状态量表(MMSE)评分、警觉-镇静(OAAS)评分均大于对照组，S-100β水平小于对照组(P＜0.05)；观察组患者胆固醇(FC)、皮质醇(Cor)、去甲肾上腺素(NE)、肾上腺素(E)水平均小于对照组(P＜0.05)；观察组患者不良反应发生率小于对照组(P＜0.05)。结论：超声引导下FICB能够改善老年髋部骨折术后患者认知功能，降低应激相关激素水平，安全性较高。

| Abstract |
[Abstract] Objective: To investigate the effects of ultrasound-guided iliac fascial space block (FICB) on postoperative cognitive function, stress related hormones and central nervous specific protein (S-100β) levels in elderly patients with hip fracture. METHODS: A total of 121 elderly patients with hip fracture in our department from March 2018 to September 2020 were selected as research objects, and they were divided into groups by drawing lot. Sixty patients in the control group were given intravenous flurbiprofen ester for postoperative analgesia, and 61 patients in the observation group were given FICB under ultrasound guidance after surgery. Cognitive function, stress-related hormones and the incidence of adverse reactions were compared between the two groups. RESULTS: The simple mental state scale (MMSE) score and vigilance-sedation (OAAS) score in observation group were higher than those in control group, and the level of S-100β was lower than that in control group (P < 0.05). The levels of cholesterol (FC), cortisol (COR), norepinephrine (NE) and epinephrine (E) in observation group were lower than those in control group (P < 0.05); The incidence of adverse reactions in observation group was lower than that in control group (P < 0.05). CONCLUSIONS: FICB under ultrasound guidance can improve cognitive function and reduce stress related hormone levels in elderly patients with hip fracture after surgery, with high safety.
