【摘要】目的:探究拉莫三嗪对慢波睡眠期持续棘慢波的良性癫痫伴中央颞区棘波(BECT)患儿认知功能、神经因子表达水平的改变。方法:选取我科93例慢波睡眠期持续棘慢波的BECT患儿作为研究对象,采用抽签法分成两组,对照组46例给予丙戊酸镁治疗,观察组47例给予联合拉莫三嗪治疗,对比两组患儿临床疗效、癫痫发作情况、认知功能、神经因子表达,以及不良反应发生情况。结果:观察组简易精神状态检测量表(MMSE)、韦氏儿童智力量表第四版(WISC-Ⅳ)评分改善幅度高于对照组,差异显著(P<0.05);观察组血清神经肽Y、胰岛素样生长因子1(IGF-1)、脑源性神经因子(BDNF)和S100B蛋白水平改善幅度高于对照组,差异显著(P<0.05);观察组癫痫发作控制情况明显优于对照组,差异显著(Z=3.1951,P<0.05);观察组总有效率明显高于对照组,差异显著(x2=4.5590,P<0.05);两组不良反应总发生率对比差异无统计学意义(P>0.05)。结论:丙戊酸镁联合拉莫三嗪治疗波睡眠期持续棘慢波BECT患儿,能够抑制神经因子异常表达,改善患儿认知功能。
Abstract
[Abstract] Objective: To investigate the effects of lamotrigine on the cognitive function and the expression level of neural factors in children with benign epilepsy with persistent chronic spines and chronic spines in the central temporal region (BECT) during slow wave sleep. Methods: to choose my families and 93 cases of slow wave sleep phases for spine slow wave of BECT patients as the research object, using the lottery method is divided into two groups, 46 cases were treated magnesium valproic acid treatment, observation group of 47 cases with given lamotrigine treatment, compared two groups of children with clinical curative effect, seizures, cognitive function and neural factor expression, and adverse reactions occur. Result: The scores of MMSE and WISC-Ⅳ in the observation group were significantly improved than those in the control group (P<0.05). Serum neuropeptide Y, insulin-like growth factor 1(IGF-1), brain-derived nerve factor (BDNF) and S100B protein levels in observation group were significantly higher than those in control group (P<0.05). The epileptic seizure control in the observation group was significantly better than that in the control group (Z=3.1951, P<0.05). The total effective rate in the observation group was significantly higher than that in the control group (X2 =4.5590, P<0.05). There was no significant difference in the total incidence of ADR between the two groups (P>0.05). Conclusion: Magnesium valproate combined with lamotrigine can inhibit the abnormal expression of nerve factors and improve the cognitive function of children with BECT during wave sleeping period.
