【摘要】探讨基于叙事理念的精益护理对经皮椎间孔镜手术围术期负性情绪及疼痛症状的影响。方法 本研究对象为2021年12月~2022年10月86例腰椎间盘突出症(LDH)患者，数字表法随机分为两组，对照组围术期采用常规护理，观察组采用基于叙事理念的精益护理。比较两组患者护理前后焦虑自评量表(SAS)、抑郁自评量表(SDS)，评估术后即刻、1d、3d时的疼痛视觉模拟评分法（VAS）。结果 两组护理后SAS评分、SDS评分均显著降低，且护理后观察组显著低于对照组，（P<0.05）；两组术后即刻、1d、3d时VAS评分逐渐降低，观察组术后即刻、1d显著低于对照组（P<0.05），术后3d时差异无统计学意义（P>0.05）。结论 基于叙事理念的精益护理能够降低LDH患者经皮椎间孔镜手术围术期焦虑、抑郁等负性情绪，减轻术后早期疼痛症状。

| Abstract |
[Abstract] Objective To explore the influence of lean nursing based on narrative concept on negative emotions and pain symptoms during perioperative period of percutaneous lumbar discectomy. Methods A total of 86 patients with lumbar disc herniation (LDH) from December 2021 to October 2022 were selected as the study subjects, and they were randomly assigned to one of two groups. The control group adopted routine nursing during perioperative period, and the observation group adopted lean nursing based on narrative concept. The self-rating anxiety scale (SAS) and self-rating depression scale (SDS) of the two groups before and after nursing were compared. The pain visual analogue scale (VAS) at immediately, 1 day and 3 days after surgery was evaluated. Results SAS score and SDS score of the two groups were significantly reduced after nursing, and the observation group was significantly lower than the control group after nursing (P<0.05). The VAS scores in the two groups decreased gradually on the immediately, 1st and 3rd day after operation, and the VAS scores in the observation group were significantly lower than those in the control group on the immediately and 1st day after operation (P<0.05). There was no significant difference 3 days after operation (P>0.05). Conclusion Lean nursing based on narrative concept can reduce the negative emotions such as anxiety and depression of patients with LDH during the perioperative period of percutaneous vertebroscopic surgery, and relieve the early postoperative pain symptoms.
