目的:基于氢质子磁共振波谱(Hydrogen Proton Magnetic Resonance Spectroscopy, 1H-MRS)检测探究成年男性精神分裂症患者杏仁核神经代谢物与阳性及阴性症状精神症状评定量表(Positive and Negative Syndrome Scale, PANSS)评分的相关性。方法:纳入2021年4月至2023年6月59例成年男性精神分裂症患者作为患者观察组,以及18例健康体检者作为健康对照组。比较两组基线资料,收集两组左右侧杏仁核N-乙酰基天门冬氨酸(N-acetylaspartate,NAA)、肌酸(Creatine,Cr)和胆碱(Choline,Cho)神经代谢物指标,比较两组Cho/Cr、NAA/Cho、NAA/(Cr+Cho)比值的差异,分析杏仁核神经代谢物指标与PANSS评分的相关性,与PANSS评分中具体各项症状评分的相关性。结果:患者组与对照组在年龄和文化程度方面差异没有统计学意义(P>0.05);患者组左侧杏仁核NAA/(Cho+Cr)、NAA/Cr、NAA/Cho、Cho/Cr高于对照组,差异具有统计学意义(P<0.05);患者组左侧杏仁核NAA/(Cho+Cr)、NAA/Cr、NAA/Cho与阴性症状评分呈负相关;患者组杏仁核NAA/Cho+Cr(L)与情感退缩(N2)、被动/淡漠(N4)、抽象思维困难(N5)、交流缺乏自发性和流畅性(N6)、刻板思维(N7)呈负相关,NAA/Cr(L)与N5、N6呈负相关,NAA/Cho(L)与N2、N4、N5、N6、N7呈负相关。结论:基于1H-MRS发现男性精神分裂症患者左侧杏仁核区NAA/(Cho+Cr)、NAA/Cr、NAA/Cho、Cho/Cr比值异常增高,可能与患者PANSS评估中阴性症状相关。

| Abstract |
Objective Based on hydrogen proton magnetic resonance spectroscopy (1H-MRS), this study aims to investigate the correlation between neurotransmitters in amygdala and the scores of the Positive and Negative Symptom Scale (PANSS) in male adults with schizophrenia. Method A total of 59 adult male patients with schizophrenia from April 2021 to June 2023 were included as the observation group, and 18 healthy individuals were included as the control group. Compare two sets of baseline data, collect N-acetylaspartate (NAA), Creatine (Cr), and Choline (Cho) neurometabolic indicators in the left and right amygdala, compare the differences in Cho/Cr, NAA/Cho, and NAA/(Cr+Cho) ratios between the two groups, analyze the correlation between amygdala neurometabolic indicators and PANSS scores, and the correlation with specific symptom scores in PANSS scores. Results There was no statistically significant difference in age and years of education between the patient group and the control group (P>0.05); The levels of NAA/(Cho+Cr), NAA/Cr, NAA/Cho, and Cho/Cr in the left amygdala of the patient group were higher than those in the control group, and the differences were statistically significant (P<0.05); The left amygdala NAA/(Cho+Cr), NAA/Cr, and NAA/Cho in the patient group were negatively correlated with negative symptom scores; The amygdala NAA/Cho+Cr (L) in the patient group is negatively correlated with emotional withdrawal (N2), passive/indifferent (N4), abstract thinking difficulties (N5), lack of spontaneity and fluency in communication (N6), and stereotyped thinking (N7). NAA/Cr (L) is negatively correlated with N5 and N6, and NAA/Cho (L) is negatively correlated with N2, N4, N5, N6, and N7. Conclusion Based on 1H-MRS, it was found that the NAA/(Cho+Cr), NAA/Cr, NAA/Cho, and Cho/Cr ratios in the left amygdala of male patients with schizophrenia were abnormally increased, which may be related to negative symptoms in PANSS score.
