血清CCL2、MIF及氧化应激指标在精神分裂症患者中的表达及意义

摘要
【摘要】目的:分析血清趋化因子2(CCL2)、巨噬细胞迁移抑制因子(MIF)及氧化应激指标在精神分裂症患者中的表达及意义。方法:选定本院2020年2月至2023年2月就诊的106例精神分裂症患者设为观察组,另选取同期本院106例健康体检者设为健康组,检测、比较两组血清CCL2、MIF水平、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)],以精神分裂症认知功能成套测验(MCCB)评估患者认知功能,比较两组MCCB评分。根据有无认知功能障碍将106例精神分裂症患者分为2组,60例伴有认知障碍者设为观察组1,46例无认知功能障碍者设为观察组2,比较两组血清CCL2、MIF水平、氧化应激指标,Pearson分析血清CCL2、MIF水平、氧化应激指标与MCCB评分的相关性,通过ROC曲线分析血清CCL2、MIF、氧化应激指标对认知功能障碍的预测价值。结果:观察组血清CCL2、MIF、MDA水平高于健康组,P<0.05,观察组血清SOD水平低于健康组,P<0.05。观察组社会认知、推理和问题解决能力、视觉学习、词语学习、工作记忆、注意/警觉性、信息处理速度评分及MMCB总分均低于健康组,P<0.05。观察组1血清CCL2、MIF、MDA水平高于观察组2,P<0.05,观察组1血清SOD水平低于观察组2,P<0.05。血清CCL2、MIF、MDA与MCCB评分均呈负相关性,P<0.05(r值=-0.394、-0.411、-0.386),血清SOD与MCCB评分呈正相关性,P<0.05(r值=0.403)。血清CCL2、MIF、SOD、MDA联合检测预测认知功能障碍的AUC是0.918,95%CI是0.785-0.952,灵敏度(94.64%)、特异度(90.67%)均高于单一检测(76.02%、72.71%)、(75.13%、71.16%)、(72.26%、69.34%)、(74.03%、70.98%),P<0.05。结论:血清CCL2、MIF、SOD、MDA异常表达参与了精神分裂症发生、发展,与患者认知功能存在密切联系,联合检测以上细胞因子可提高对认知功能障碍的预测灵敏度、特异度,具有一定的参考价值。
Abstract
【Abstract】Objective: To analyze the expression and significance of serum chemokine 2 (CCL2), macrophage migration inhibitor (MIF) and oxidative stress index in patients with schizophrenia. Methods: A total of 106 patients with schizophrenia who received medical treatment in our hospital from February 2020 to February 2023 were selected as the observation group, and 106 healthy patients who underwent physical examination in our hospital during the same period were selected as the healthy group. Serum levels of CCL2, MIF and oxidative stress indexes [superoxide dismutase (SOD) and malondialdehyde (MDA)] of the two groups were detected and compared. Cognitive function was assessed by schizophrenia cognitive function Kit test (MCCB), and MCCB scores were compared between the two groups. 106 patients with schizophrenia were divided into 2 groups according to whether they had cognitive impairment. 60 patients with cognitive impairment were set as the observation group, and 46 patients without cognitive impairment were set as the observation group. Serum levels of CCL2, MIF and oxidative stress indexes were compared between the two groups. Pearson was used to analyze the correlation between serum CCL2, MIF levels, oxidative stress indexes and MCCB scores, and ROC curve was used to analyze the predictive value of serum CCL2, MIF and oxidative stress indexes on cognitive dysfunction. Results: The serum levels of CCL2, MIF and MDA in the observation group were higher than those in the healthy group, P < 0.05, and the serum level of SOD in the observation group was lower than that in the healthy group, P < 0.05. Social cognition, reasoning and problem solving ability, visual learning, word learning, working memory, attention/alertness, information processing speed score and total score of MMCB in observation group were lower than those in healthy group (P < 0.05). The serum levels of CCL2, MIF and MDA in observation group 1 were higher than those in observation group 2 (P < 0.05), and the serum level of SOD in observation group 1 was lower than that in observation group 2 (P < 0.05). Serum CCL2, MIF and MDA were negatively correlated with MCCB score, P < 0.05 (r =-0.394, -0.411, -0.386), and serum SOD was positively correlated with MCCB score, P < 0.05 (r =0.403). The AUC of the combined detection of serum CCL2, MIF, SOD and MDA was 0.918, and the 95%CI was 0.785-0.952. The sensitivity (94.64%) and specificity (90.67%) were higher than that of single detection (76.02%, 72.71%), (75.13%, 71.16%), (72.26%, 69.34%), (74.03%, 70.98%), P < 0.05. Conclusion: Abnormal expression of CCL2, MIF, SOD and MDA in serum is involved in the occurrence and development of schizophrenia, which is closely related to the cognitive function of patients. Combined detection of these cytokines can improve the sensitivity and specificity of the prediction of cognitive dysfunction, which has certain reference value.
