目的:探讨急性期精神分裂症患者非酶类抗氧化物水平与攻击行为的关系。方法:纳入急性期精神分裂症患者162例,其中非攻击组77例,攻击组85例。采用自制的调查问卷收集患者的一般社会资料,采用阳性和阴性症状量表评估患者的临床精神症状,采用修改版外显攻击行为量表评估患者的攻击行为,使用全自动生化分析仪检测患者的非酶类抗氧化物水平,对非酶类抗氧化物水平与攻击行为的关系进行统计学分析。结果:攻击组的阳性症状分高于非攻击组(P<0.05),攻击组的总胆红素(total bilirubin,TBIL)、直接胆红素、间接胆红素水平均高于非攻击组(P<0.05)。性别、使用氯氮平、TBIL水平、阳性症状分是急性期精神分裂症患者攻击行为的影响因素,其中TBIL水平影响最为显著(OR=1.275,P<0.001)。结论:急性期伴有攻击行为的精神分裂症患者体内存有非酶类抗氧化物水平变化,其中胆红素水平差异尤为显著,并且高TBIL水平是攻击行为的危险因素。

| Abstract |
Objective: To explore relationship between levels of non-enzymatic antioxidants and aggressive behavior in patients with acute schizophrenia. Methods: A total of 112 patients with acute schizophrenia were enrolled, including 77 patients in non-aggressive group and 85 patients in aggressive group. General social data of patients were collected by self-made questionnaire, clinical symptoms were assessed by the Positive and Negative Symptom Scale, aggressive behavior were assessed by the Modified Overt Aggression Scale, and non-enzymatic antioxidants levels were detected by fully automated biochemical analyzer. The relationship between levels of non-enzymatic antioxidants and aggressive behavior were statistically analyzed. Results: The positive symptom score in the aggressive group was higher than that in the non-aggressive group (P<0.05), while the levels of total bilirubin (TBIL), direct bilirubin, and indirect bilirubin in the aggressive group were higher than those in the non-aggressive group (P<0.05). Gender, use clozapine, TBIL, and positive symptom score are the influencing factors of aggressive behavior in patients with acute schizophrenia, and high TBIL level having the most significant impact (OR=1.275,P<0.001). Conclusion: There are some changes in levels of non-enzymatic antioxidants in patients with acute schizophrenia accompanied by aggressive behavior, the difference in bilirubin level is particularly significant, and high TBIL level is a risk factor for aggressive behavior.
