Objective This paper mainly studies the management of chronic diseases and analyzes the application value of community nursing services. Methods In January 2023-January 2024 I community with diabetes, hypertension and other chronic diseases with 100 patients as a research object, in view of the research needs, the patients group, 1 / 2 patients into the reference group and give their regular chronic disease management care, another 1 / 2 patients into the research group and give the community care service chronic disease management, observe two groups of nursing effect, and statistical patient quality of life score, self-management ability, patient satisfaction research data, data comparative analysis by statistical methods, to determine nursing difference is effectiveness. Results After the statistical comparison of the two groups, there was no significant difference in patients quality of life score and self-management ability level (P> 0.05), and the two groups were significantly different (P <0.05); for the comparison of nursing satisfaction, the total satisfaction rate of the study group was higher than the reference group, and the difference was obvious (P <0.05). Conclusion The application value of community nursing service in community chronic disease management is high, which is helpful to improve the life of the quality of patients with chronic diseases, improve the level of patients self-management ability, obtain the satisfaction of patients, and has high application value.
