Objective To evaluate the effect of continuous dynamic follow-up care on hypertension patients with coronary heart disease under the concept of general practice, including blood pressure control, cardiovascular event incidence, quality of life and so on. Methods 80 patients admitted to our hospital within 1 year from January 2023 to January 2024 were selected, and all patients were randomly assigned into control group (40 cases, receiving routine care) and observation group (40 cases, establishing close contact between general practitioners and patients). The effects of the two groups of patients after the completion of nursing were collected and analyzed. Results The blood pressure control rate, cardiovascular event rate, quality of life score, medication compliance, readmission rate and unplanned visit rate of the observation group were significantly better than those of the control group. Conclusion Continuous dynamic follow-up nursing under the general practice concept has obvious advantages in the management of hypertension combined with coronary heart disease, which can significantly improve the blood pressure control rate, reduce the incidence of cardiovascular events, improve medication compliance, and reduce the readmission rate and unplanned visit rate.
