肝癌患者经济毒性影响因素分析及临床应对策略

摘要	研究肝癌患者经济毒性（financial toxicity，FT）的影响因素，为精准识别高危人群提供依据。方法选取2023年6月~12月期间我院肝胆外科收治的265例肝癌患者为研究对象。运用一般资料调查表、领悟社会支持量表（PSSS）、心理弹性量表（CD-RISC）和经济毒性综合评分表（COST-PROM）进行问卷调查。通过单因素分析和普通最小二乘法（Ordinary Least Squares，OLS）多因素回归分析，筛选出肝癌患者FT的影响因素。结果肝癌患者COST-PROM得分为22分（8~42分），平均得分为（22.94±6.33）分。文化程度、家庭月收入、因病失业、患病时长、治疗史是肝癌患者FT的主要影响因素。结论对肝癌患者加强健康教育和医疗知识普及、加强心理疏导和社会关怀、医疗机构提供经济合理的治疗方案等，可以降低肝癌患者的FT水平。
Abstract	Objective To study the influence factors of financial toxicity (FT) in patients with liver cancer, so as to provide basis for accurate identification of high-risk groups. Methods A total of 265 patients with liver cancer admitted to the Department of Hepatobiliary Surgery in our hospital from June to December 2023 were selected as the study objects. General data questionnaire, Perceptive Social Support Scale (PSSS), Connor-Davidson Resilience Scale (CD-RISC) and Comprehensive Scores for Financial Toxicity based on the Patient-reported Outcome Measures (COST-PROM) were used to conduct questionnaire survey. Through univariate analysis and Ordinary Least Squares (OLS) multivariate regression analysis, the influencing factors of FT in patients with liver cancer were screened. Results The COST-PROM scores of patients with liver cancer were 22 (8-42), and the average score was (22.94±6.33). Education level, monthly family income, unemployment due to illness, duration of illness and treatment history are the main influencing factors of FT in patients with liver cancer. Conclusion Strengthening health education and medical knowledge popularization, strengthening psychological counseling and social care, and providing economic and reasonable treatment plan in medical institutions can reduce the FT level of patients with liver cancer.
