| 摘要 |
本研究旨在评估老年高血压糖尿病患者护理中全程健康教育模式的实施效果。方法 选择我院于2023年1月至2024年1月期间纳入68例老年高血压糖尿病患者作为研究对象，随机分为对照组（34例）和观察组（34例）。对照组接受传统护理模式，包括常规医疗指导和治疗；观察组接受全程健康教育模式，包括定期的个性化健康教育、营养指导、运动建议等。结果 观察组患者在血压和血糖控制方面表现出显著改善，生活质量得到提高，与对照组相比具有统计学意义。结论 老年高血压糖尿病患者护理中全程健康教育模式的实施能够有效改善患者的血压、血糖控制情况，提高生活质量，为临床实践提供了重要参考依据。

| Abstract |
Objective This study was to evaluate the effect of the whole-course health education model in nursing care of elderly patients with hypertension and diabetes. Methods: A total of 68 elderly patients with hypertension and diabetes were enrolled in our hospital from January 2023 to January 2024, and randomly divided into control group (34 cases) and observation group (34 cases). The control group received traditional nursing mode, including conventional medical guidance and treatment; The observation group received the whole-course health education mode, including regular personalized health education, nutrition guidance, exercise advice, etc. Results: Patients in the observation group showed significant improvement in blood pressure and blood glucose control and improved quality of life, which was statistically significant compared with the control group. Conclusion: The implementation of whole-course health education model in nursing care of elderly patients with hypertension and diabetes can effectively improve the control of blood pressure and blood sugar of patients, improve the quality of life, and provide an important reference for clinical practice.
