| 摘要 |
分析在社区老年高血压中健康教育和慢性病管理的效果。方法 在选择2020年至2021年在社区诊断和治疗的80岁高血压患者时，分为观察组（健康教育和慢性病管理）和对照组（常规护理）。测量护理前后两组患者的血压值、护理后的生活质量以及患者服药依从性进行比较。结果 观察组患者护理后的血压水平改善情况明显优于对照组，生活质量优于对照组,服药依从性高于对照组（P<0.05）。结论 健康教育和慢性病管理效果明显，值得广泛推广与应用。

| Abstract |
Objective To analyze the effectiveness of health education and chronic disease management in elderly hypertensive patients in the community. Methods When selecting 80-year-old hypertensive patients diagnosed and treated in the community from 2020 to 2021, they were divided into an observation group (health education and chronic disease management) and a control group (routine care). Compare the blood pressure values, quality of life after nursing, and medication adherence of two groups of patients before and after nursing care. Results The improvement in blood pressure level and quality of life of the observation group patients after nursing care was significantly better than that of the control group, and their medication adherence was higher than that of the control group (P < 0.05). Conclusion Health education and chronic disease management have significant effects and are worthy of widespread promotion and application.
