A case of 30-year-old male patient with schizophrenia was treated with Olanzapine 20mg once per night at the first time of hospitalization, and was discharged after recovery without extra-pyramidal syndrome. At the second time of hospitalization, donepezil was given orally 5mg once per night for his decreased cognitive function and memory. After 5 days, extra-pyramidal syndrome such as mechanical movements and dull facial expressions appeared, so the patient was given 2 days of treatment with trihexyphenidyl 2mg 3 times daily, then the symptoms disappeared. After that, donepezil and trihexyphenidyl were gradually stopped and olanzapine tablets were continued, and extrapyramidal syndrome did not appear again. The patient was followed up for half a year after discharge and his conditions were stable. At the same time, no side effects such as extrapyramidal syndrome were observed.
