摘要
物质成瘾是一种慢性复发性脑疾病,成瘾者通常会陷入从耐受到渴求直至复吸的过程。其机制被认为是多巴胺释放增多产生的奖赏兴奋效应。成瘾后,患者个人身心健康,家庭及社会健康发展都将受到负面影响。目前的常规治疗手段都存在不同程度的缺陷。近年来,非侵入式神经调控技术因其无痛无创、易耐受、便捷安全等优势成为治疗物质成瘾的研究热点。据己有研究表明,tDCS可利用恒定、低强度电流调节大脑皮层神经元活动,对于成瘾性疾病有一定的治疗作用。经TMS治疗后的物质成瘾患者,焦虑心态、渴望程度、消极情绪等均有所改善。TUS能够聚焦到脑深部发挥作用,相较前两种技术具有更高的空间准确度和时间分辨率。三种技术均可调节大脑皮层兴奋性和中脑边缘系统多巴胺的释放,对物质依赖起到较好治疗效果。本文章就非侵入性脑调控技术治疗物质成瘾的疗效机制和具体应用作出介绍。
Abstract
Substance addiction is a chronic recurrent brain disease. Addicts usually evolve from accidental use to forced use. The adaptive change of the central nervous system makes individuals fall into the process from tolerance to craving and continuous relapse. The withdrawal process is very difficult. With the continuous progress of medical technology, non-invasive neural regulation technology is widely used in the clinical rehabilitation and treatment of various neuropsychiatric diseases, and has gradually become a potential method and means to treat addictive behavior. According to previous studies, tDCS can regulate the activity of cortical neurons by using constant and low-intensity current, which has a certain therapeutic effect on addictive diseases. After TMS treatment, the anxiety, desire and negative emotion of substance addicts were improved. This paper introduces the neural mechanism and normative methods of non-invasive brain regulation technology in the treatment of substance addiction.
