系统评价ECT治疗抑郁症患者后IL-6水平的变化及其与疗效的关系。方法计算机检索中国知网、中国生物医学文献数据库(SinoMed)、万方、维普;外文数据库有:PUBMED、Web of science、Cochrane library、EMBASE,根据纳入、排除标准,两名研究者独立筛选文献、质量评价和资料提取,应用Stata软件进行Meta分析。结果 最终纳入13个研究(n=470),Meta结果显示,重复ECT治疗后患者IL-6水平较前下降(随机效应模型,SMD=0.49,95% CI(0.11,0.88)),单次ECT后患者IL-6水平升高(固定效应模型,SMD=-0.58,95% CI(-1.02,-0.15))。结论 本系统评价结果显示单次ECT使得IL-6水平急性短暂诱导升高,重复ECT治疗后外周血IL-6水平会逐渐降低。ECT能调节抑郁症患者紊乱的神经免疫功能,单次ECT短期促进炎症,而长期重复ECT具有抗炎作用。

| Abstract |
Objective To systematically evaluate the changes of IL-6 levels during the electroconvulsive treatment in depressed patients and the relationship with efficacy.Methods We searched in CNKI, SinoMed, Wanfang, VIP databases, foreign language databases include PUBMED, Web of science, Cochrane library, EMBASE. Based on the preset inclusion and exclusion criteria, documents screening, quality assessment of methodology, data extraction were conducted by two researchers separately. We used STATA software for data analysis.Results Thirteen studies ( n = 470 subjects) were eligible. The results showed that IL-6 levels decreased after repeated ECT (random-effect model, SMD=0.49,95% CI (0.11,0.88)), and IL-6 levels increased after single ECT (fixed-effect model, SMD= -0.58,95% CI (-1.02, -0.15)).Conclusion The systematic evaluation showed that a single ECT induced IL-6 levels briefly, while peripheral blood IL-6 levels gradually decreased after repeated ECT.ECT can regulate the disordered neuroimmune function in depressed patients.Single ECT promotes inflammation in the short term, while long term repeat ECT has anti-inflammatory effects..
