伴与不伴精神病性症状青少年重度抑郁发作抑郁及焦虑症状比较

【摘要】 以重度抑郁发作青少年患者为研究对象，探讨伴与不伴精神病性症状的重度抑郁发作抑郁和焦虑症状的差异。方法 纳入2015年－2021年在深圳市康宁医院住院部就诊、符合《国际疾病分类（第10版）》（ICD－10）重度抑郁发作诊断标准的青少年176例，采用宗氏抑郁量表（SDS）和焦虑自评量表(SAS)进行评定。结果 伴有精神病性症状组负性情绪、低自尊和躯体症状的得分均高于不伴精神病性症状组（t=2.039，P=0.043）、(t=2.050，P=0.042)、(t=2.025，P=0.044）；伴有精神性症状组相较于不伴精神病性症状组负性情绪、低自尊、社会焦虑和焦虑量表总分达到重度，差异具有统计学意义（²=5.550，P=0.018)、(²=7.540，P=0.006)、(²=7.424，P=0.006 )、（²=5.043，P=0.025）结论 伴有精神病症状的重度抑郁障碍的青少年相较于不伴精神病性症状的青少年更容易表现负性情绪、低自尊和躯体症状，负性情绪、低自尊、社会焦虑和焦虑症状更为严重。

| Abstract |
【Abstract】 Objective To explore the difference of depression and anxiety symptoms between adolescents with severe depression and those without psychotic symptoms. Methods 176 adolescents who were hospitalized in Shenzhen Mental Health Center from 2015 to 2021 and met the diagnostic criteria of major depressive disorder in the International Classification of Diseases，tenth edition（ICD－10）criteria were assessed by Self－rating depression scale (SDS) and Self－Rating Anxiety Scale (SAS). Result The scores of negative emotion, low self－esteem, low self－esteem and somatic symptoms in the group with psychotic symptoms were higher than those in the group without psychotic symptoms （t=2.039，P=0.043）(t=2.050,P=0.042)(t=2.025,P=0.044）. and the scores of negative emotion, low self－esteem, social anxiety and anxiety scale in the group with psychotic symptoms were higher than those in the group without psychotic symptoms, and the difference was statistically significant（²=5.550，P=0.018)(²=7.540，P=0.006)(²= 7.424，P=0.006 )（²=5.043，P=0.025）. Conclusion Adolescents with severe depressive disorder with psychiatric symptoms are more likely to exhibit negative emotions, low self－esteem and physical symptoms, and have more severe symptoms of negative emotions, low self－esteem, social anxiety and anxiety than adolescents without psychiatric symptoms.
