双相情感障碍患者甲状腺功能的性别差异及其相关性分析

| 摘要 |
【摘要】分析双相情感障碍(bipolar disorder,BD)患者甲状腺功能的性别差异及其相关性研究。方法 根据性别将BD患者分男性94例和女性38例两组，收集其治疗前后的临床资料和甲状腺功能指标。结果 治疗前两组患者住院时间、游离三碘甲状腺原氨酸(tree triiodothyronine,FT3)、总三碘甲状腺原氨酸(total triiodothyronine,TT3）及抗抑郁药的使用上差异有统计学意义(P<0.05)；治疗前后总甲状腺素(total thyroxine,TT4)、游离甲状腺素(free tetraiodothyronine,FT4)差值组间差异有统计学意义(P<0.05)；男性中FT4差值与年龄呈正相关，发病年龄与FT4、TT4差值呈正相关，精神病性症状与FT3、FT4、TT4差值呈负相关；而女性中TT4差值与精神病性症状呈正相关。结论 BD患者治疗前后的甲状腺功能存在性别差异，男性中甲状腺功能与年龄、发病年龄、精神病性症状相关，女性中甲状腺功能与精神病性症状相关。

| Abstract |
【Abstract】Objective To analyze the gender difference and related factors of thyroid function in patients with bipolar disorder(BD).Methods According to gender, BD patients were divided into two groups (male=94， female=38). The clinical data and thyroid function indexes were collected at the beginning and end of treatment. Results At the beginning of treatment, there were significant differences in hospital stay, free triiodothyronine (FT3),total triiodothyronine (TT3) and the use of antidepressants between the two groups (p<0.05); there were significant differences in total thyroxine (TT4) and free thyroxine (FT4) at the beginning and end of treatment (p<0.05); In male group, decreased score before and after treatment of FT4 was positively correlated with age, the age of onset was positively correlated with the decreased score of FT4 and TT4, and the psychotic symptoms were negatively correlated with the decreased score of FT3, FT4 and TT4; the decreased score of TT4 was positively correlated with psychotic symptoms in female group. Conclusion There are gender differences in thyroid function in BD patients at the beginning and end of treatment. The thyroid function in males is related to age, onset age and psychotic symptoms, while in female, thyroid function is related to psychotic symptoms.
