【摘要】探讨间歇性Theta节律刺激(iTBS)对脑梗死抑郁合并认知功能障碍的干预效果。方法 本研究对象为2020年1月~2022年1月我院120例脑梗死抑郁合并认知功能障碍患者,采用数字表法随机分为两组。对照组采用常规治疗,观察组在常规治疗基础上给予iTBS治疗,共治疗6周。治疗前、治疗后采用简易精神状态量表(MMSE)、改良的Barthel指数评定表(MBI)、美国国立卫生研究院卒中量表(NHISS)分别评估认知功能、活动能力以及神经功能,采用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评估患者抑郁、焦虑情绪。结果 两组干预后HAMA评分、HAMD评分均降低,且干预后观察组显著低于对照组,(P<0.05);两组干预后MMSE评分、MBI评分均升高,NHISS评分下降,且治疗后观察组各项指标均优于对照组(P<0.05)。结论 iTBS能显著降低脑梗死抑郁合并认知功能障碍患者焦虑、抑郁情绪,提升认知功能,并能提升神经功能以及活动能力。
Abstract
[Abstract] Objective To investigate the intervention effect of intermittent Theta rhythm stimulation (iTBS) on cerebral infarction depression complicated with cognitive dysfunction. Methods A total of 120 patients with cerebral infarction and depression combined with cognitive impairment in our hospital from January 2020 to January 2022 were the subjects of this study and randomly divided into two groups by the digital table method. The control group was given conventional treatment, and the observation group was given iTBS treatment on the basis of conventional treatment for a total of 6 weeks. Before and after treatment, the Mini Mental State Scale (MMSE), Modified Barthel Index (MBI) and National Institutes of Health Stroke Scale (NHISS) were used to evaluate cognitive function, activity ability and neurological function, respectively. The Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) were used to evaluate the depression and anxiety of the patients. Results The HAMA scores and HAMD scores of the two groups were decreased after the intervention, and the observation group was significantly lower than the control group after the intervention (P<0.05). After intervention, the MMSE score and MBI score in the two groups were increased, and the NHISS score was decreased, and the indexes in the observation group were better than those in the control group after treatment (P<0.05). Conclusion iTBS can significantly reduce anxiety and depression in patients with cerebral infarction and depression combined with cognitive dysfunction, improve cognitive function, and improve neurological function and activity.
