米氮平对脑外伤术后脑脊液漏伴颅内感染患者焦虑、抑郁及5-HT水平的影响机制探讨

【摘要】目的：探究米氮平应用于脑外伤术后脑脊液漏伴颅内感染患者对其焦虑、抑郁及5-羟色胺(5-HT)水平的影响。方法：选取我院2018年8月至2020年12月期间107例脑外伤术后脑脊液漏伴颅内感染患者作为研究对象，随机数字表法分两组，对照组53例给予单纯腰大池置管持续引流治疗，观察组54例增加米氮平治疗，对比两组患者临床疗效，认知功能、焦虑抑郁程度、5-HT水平，及不良反应发生情况。结果：观察组总有效率高于对照组（P＜0.05）；观察组认知功能评定量表(LOTCA)各项评分均高于对照组；观察组焦虑自评量表(SAS)及抑郁自评量表（SDS）评分均较对照组低；观察组5-HT水平高于对照组（均P＜0.05）；两组不良反应比较差异无统计学意义（P＞0.05）。结论：脑外伤术后脑脊液漏伴颅内感染患者采用米氮平与单纯腰大池置管持续引流联合治疗效果显著，可有效改善患者认知功能，调节5-HT水平，缓解焦虑抑郁情绪。

| Abstract |
[Abstract] Objective: To investigate the effects of mirtazapine on anxiety, depression and 5-hydroxytryptamine (5-HT) levels in patients with cerebrospinal fluid leakage and intracranial infection after traumatic brain injury. Methods:From August 2018 to December 2020, 107 patients with cerebrospinal fluid leakage and intracranial infection after traumatic brain injury in our hospital were selected as the research objects. They were divided into two groups by random number table method. 53 patients in the control group were treated with continuous drainage of lumbar cistern alone, and 54 patients in the observation group were treated with mirtazapine. Cognitive function, anxiety and depression degree, 5-HT level, and incidence of adverse reactions. Results: The total effective rate of observation group was higher than that of control group (P < 0.05). The LOTCA scores of the observation group were higher than those of the control group. The scores of self-rating Anxiety Scale (SAS) and self-rating depression Scale (SDS) in the observation group were lower than those in the control group. The level of 5-HT in observation group was higher than that in control group (ALL P < 0.05). There was no significant difference in adverse reactions between the two groups (P > 0.05). Conclusion: Mirtazapine combined with continuous lumbar cistern drainage can effectively improve the cognitive function, regulate the level of 5-HT and relieve anxiety and depression in patients with cerebrospinal fluid leakage and intracranial infection after traumatic brain injury.
