Objective This article mainly for continuity of care and routine care to explore, observe the clinical effect of differential care after hip and knee replacement, and determine the impact on the prognosis of patients. Methods The author included 100 cases of patients who underwent hip and knee replacement treatment in our hospital from January 2022 to January 2023 to participate in this study, evenly divided into two groups, the continuity of care was applied to the study group of patients (50 cases), conventional care was applied to the reference group of patients (50 cases), the clinical effect was observed and the data of each group were compared between groups. Results There was no difference in hip function scores and knee function scores one month after surgery (P > 0.05). There were differences in joint function scores between the two groups at 2-3 months after surgery (P < 0.05). Postoperative 1-3 joint pain scores were lower in the study group and higher in the reference group (P < 0.05). The scores of physical function, physiological function, mental function, mental state and other quality of life in the study group were higher than those in the control group (P < 0.05). The total satisfaction rate in the study group was higher than that in the reference group (P < 0.05). Conclusion For patients after hip and knee replacement respectively carry out continuity of care and routine care, continuity of care effect is more obvious, patients with postoperative joint function, pain are rapidly improved, to improve the quality of life of patients play a very big role, the effect is recognized by patients, clinical has a wide range of application significance.
