Objective To explore the application effectiveness of the Triangle Model in the continuity of care for peritoneal dialysis patients. Methods From October 2022 to October 2023, 220 patients were screened at our hospitals peritoneal dialysis center, with 120 PD patients meeting the inclusion and exclusion criteria being selected for the study and randomly divided into an observation group and a control group, each comprising 60 cases. The control group received standard nursing patterns, while the observation group received measures for continuity of care. Results In terms of nursing satisfaction, the observation group demonstrated 96.66%, whereas the control group showed 78.33%, indicating a significant difference with statistical significance (P < 0.05). Concerning the psychological condition, the observation group showed post-nursing SAS and SDS scores of (24.69±2.10) and (25.13±1.87) respectively, significantly lower than those of the control group, with statistical significance (P < 0.05). Conclusion The implementation of continuity of care measures based on the Triangle Model in the nursing work of peritoneal dialysis patients indicates enhanced patient compliance with nursing procedures, fostering emotional stability during the course of care and consequently improving the quality of life for patients, presenting noteworthy implications.
